摘要
目的探讨转移抑制基因nm23及结核菌L型感染与前列腺癌发病的相关性。方法选取2007年1月-2017年1月医院收治的前列腺癌患者580例作为研究对象。通过免疫组织化学检验,观察nm23蛋白及nm23mRNA在不同分期前列腺癌患者中的表达情况,记录所有前列腺癌患者中结核菌L型感染概率,分析结核菌L型感染与前列腺癌的关系。结果 580例前列腺癌患者中,nm23蛋白表达阳性患者共297例(51.21%),nm23mRNA阳性检出301例(51.90%);随病理分级和临床分期增高,nm23蛋白、mRNA的阳性检出率逐渐降低,结核菌L型感染检出率逐渐增高(P<0.05);580例患者中,有213例检出前列腺癌转移,非转移患者nm23蛋白、nm23mRNA的阳性检出率高于转移患者(P<0.05);非转移组结核菌L型感染检出率低于转移组(P<0.05)。结论 nm23蛋白在前列腺肿瘤中表达情况受病理分级和临床分期影响,提示nm23蛋白可作为评估前列腺癌患者肿瘤分化程度和预后的重要指标。随前列腺癌病情进展,结核菌L型感染发生风险增加,提示结核菌L型感染可能影响前列腺癌的发生和进展,值得临床进一步研究。
        OBJECTIVE To explore the metastasis suppressor gene nm23 and the correlation between Mycobacterium tuberculosis L-form infection and pathogenesis of prostate cancer.METHODS A total of 580 prostate cancer patients who were treated in hospitals from Jan 2007 to Jan 2017 were recruited as the study objects.The expression of nm23 protein and nm23 mRNA in the patients with different stages of prostate cancer was observed by means of immunohistochemistry test,the probabilities of the M.tuberculosis L-form infection were recorded,and the correlation between the M.tuberculosis L-form infection and the prostate cancer was analyzed.RESULTS Of the 580 prostate cancer patients,297(51.21%)were tested positive for nm23 protein,and 301(51.90%)were tested positive for nm23 mRNA.With the increase of tumor pathological grade and clinical stage,tjhe positive rates of nm23 protein and mRNA were gradually decreased,while the detection rate of M.tuberculosis L-form infection was gradually increased(P<0.05).Among the 580 patients,213 were diagnosed with prostate cancer metastasis;the positive rates of nm23 protein and nm23 mRNA of the patients without metastasis were significantly higher than those of the patients with metastasis(P<0.05);the incidence of Mt.uberculosis L-form infection was lower in the patients without metastasis than in the patients with metastasis(P<0.05).CONCLUSIONThe expression of nm23 protein is affected by the pathological grade and clinical stage,suggesting that nm23 protein can be used as an important index for evaluation of the degree of tumor differentiation and prognosis of the prostate cancer.The risk of M.tuberculosis L-form infection is increased with the progress of prostate cancer,indicating that the M.tuberculosis L-form infection may affect the occurrence and progression of prostate cancer,and it is worthy to be promoted in the hospital.
