人工全膝关节置换术患者手术等待期焦虑水平的调查分析

摘要
目的评估人工全膝关节置换术(TKA)患者手术等待期的焦虑状况。方法选取择期行TKA患者121例,采用状态焦虑问卷(S-AI)对手术前晚、手术日晨和入手术室3个时间点的焦虑水平进行调查。比较患者不同时间点生命体征及S-AI评分,以及首台与接台患者不同时间点的S-AI评分。结果入手术室时,患者的脉率、收缩压、舒张压及S-AI评分均高于手术前晚和手术日晨,差异有统计学意义(P<0.01);接台患者手术前晚、手术日晨及入手术室时S-AI评分均高于首台患者,差异有统计学意义(P<0.01)。结论 TKA患者手术等待期焦虑水平随着手术时间的临近而逐渐升高,且接台患者发生焦虑的风险高于首台患者。
Objective To evaluate the anxiety status of patients undergoing total knee arthroplasty(TKA) in waiting period. Methods Totally 121 patients selectively undergoing TKA were collected, and their anxiety status was surveyed by stateanxiety inventory(S-AI) in the night before the operation, in the operation morning and at the time of entering into operation room. The vital signs and S-AI points among patients as well as S-AI points between the patients undergoing the first operation and those undergoing consecutive operation were compared at different time points. Results When compared with the night before the operation and the operation morning, the pulse frequency, systolic pressure, diastolic pressure and S-AI scores were all elevated among patients at the time of entering in to operation room, and the significant differences were shown(P<0.01). S-AI points in patients undergoing consecutive operation were markedly higher than those in patients undergoing the first operation, and the significant differences were shown(P<0.01). Conclusion The anxiety level in patients undergoing TKA increases gradually in waiting period as the operation time approaches, and the risk of anxiety in patient undergoing consecutive operation is higher than those undergoing the first operation.
