摘要
目的总结健康素养与预防性医疗保健服务、门诊服务、住院服务和急诊服务利用关系的研究进展。方法在Pub Med、Web of Science、CNKI、万方和维普数据库,以"健康素养"、"医疗保健服务"、"预防性医疗保健服务"、"门诊服务"、"住院服务"、"急诊服务"、"利用"为主题词或关键词,同时辅以手工检索和文献追溯检索近5年相关文献。着重整理各类医疗保健服务利用的具体衡量指标,不同特征群体的健康素养与医疗保健服务利用衡量指标关系的研究结论,以及两者间的潜在中介变量。按照医疗保健服务利用的4个方面对资料进行汇总并撰写综述。结果检索并阅读相关文献70余篇。结论健康素养与特殊群体(老年人、产妇)的预防性医疗保健服务利用以及部分慢性病患者的住院和急诊服务利用相关;与门诊服务利用的关系尚无一致结论;疾病相关知识和药物依从性、自我照护和医疗保健可及性可能是健康素养影响医疗保健服务利用的中介变量。
        Objective To summarize the research progress in the relationship between health literacy and use of preventive care,visits to clinics,use of hospitalizations and emergency services,respectively. Methods The related literatures in the past five years were retrieved with subjects or keywords "health literacy","healthcare utilization","preventive care","outpatient physician services","hospitalization","emergency services","utilization"in Pub Med,Web of Science,CNKI,Wan Fang and Chongqing VIP database. Manual retrieval and literature tracing were used for supplement. The indicators of various healthcare utilization,the research conclusions of the relationships between health literacy and indicators of various healthcare utilization in groups with different characteristics,and the potential mediators between the two aspects were emphatically sorted out. Data was summarized and the article was written in accordance with four aspects of the healthcare utilization. Results More than 70 articles were searched and read. Conclusion Health literacy is related to the use of preventive care among special groups( the elderly and the maternal) and associated with the use of hospitalizations and emergency services among some patients with chronic diseases. There is no consistent conclusion in the association between health literacy and visits to clinics. The possible mediators of the impacts of health literacy on healthcare utilization include disease-related knowledge and medication adherence,self-care and access to medical care.
