胰头肿块型慢性胰腺炎的CT诊断与鉴别诊断

摘要
目的:探讨胰头肿块型慢性胰腺炎的CT特征,并与胰头癌进行鉴别诊断。方法:选取胰头肿块型慢性胰腺炎确诊患者60例(胰腺炎组)和胰头癌确诊患者50例(胰头癌组),对两组患者均多层螺旋CT检查影像学结果进行对照分析。结果:胰腺炎组患者门静脉期CT值明显高于胰头癌组(P<0.05),但两组动脉期强化CT值之间并无明显差异(P>0.05);胰腺炎组患者胰管扩张、病灶远端胰管串珠样扩张、胰管钙化率均明显高于胰头癌组,其病灶处胰管截断率、病灶远端胰管平滑扩张率、胆总管扩张率、病灶远端胆总管截断率均明显低于胰头癌组(P<0.05)。结论:CT检查可有效反应胰头肿块型慢性胰腺炎患者胰管、胆总管形态特征,可作为胰头肿块型慢性胰腺炎和胰头癌鉴别诊断的重要依据。
Objective To investigate the CT features of chronic pancreatitis with pancreatic head mass and to differentiate it from pancreatic head cancer.Method 60 patients with pancreatic head mass chronic pancreatitis(pancreatitis group)and 50 patients with pancreatic head cancer(pancreatic head cancer group)were selected.The imaging results of multi-slice spiral CT in both groups were compared and analyzed.Result The CT value of portal vein phase in pancreatitis group was significantly higher than that in pancreatic head cancer group(P<0.05),but there was no significant difference between the two groups(P>0.05).The pancreatic duct dilatation,distal pancreatic duct bead dilatation and pancreatic duct calcification rates in pancreatitis group were significantly higher than those in pancreatic head cancer group,and the rates of pancreatic duct truncation,smooth dilatation of distal pancreatic duct,common bile duct dilatation and lesion were significantly higher than those in pancreatic head cancer The rate of distal common bile duct truncation was significantly lower than that of pancreatic head cancer group(P<0.05).Conclusion CT examination can effectively reflect the morphological characteristics of pancreatic duct and common bile duct in patients with pancreatic head mass type chronic pancreatitis,and can be used as an important basis for differential diagnosis of pancreatic head mass type chronic pancreatitis and pancreatic head cancer.
