麻醉前集中护理管理对扁桃体及腺样体切除术病人应激反应及负性情绪的影响

摘要
[目的]观察麻醉前集中护理管理对扁桃体及腺样体切除术病人围术期应激反应及负性情绪的影响。[方法]选取2017年1月—2017年12月本院收治的124例扁桃体及腺样体切除术病人,根据随机数字表法将124例病人分为观察组及对照组各62例,对照组手术当天入室前行常规健康教育,观察组于手术当天入室前行麻醉前集中管理,比较两组病人术前1d、麻醉前及术后1d心理应激反应、负性情绪及麻醉苏醒时间、并发症情况。[结果]观察组麻醉前、术后1d焦虑自评量表(SAS)及抑郁自评量表(SDS)评分均低于对照组(P<0.05)。观察组麻醉前、术后1d心率、收缩压、舒张压、前列环素水平均低于对照组(P<0.05)。观察组麻醉苏醒时间、拔管时间短于对照组(P<0.05),躁动、认知功能障碍发生率低于对照组(P<0.05)。[结论]术前实施集中护理管理能有效减轻扁桃体及腺样体切除术病人围术期应激反应及负性情绪,有利于提高病人麻醉苏醒质量及安全性。
Objective:To observe the effects of centralized nursing management before anesthesia on perioperative stress response and negative emotion in patients with tonsillectomy and adenoidectomy.Methods:A total of 124 patients with tonsillectomy and adenoidectomy admitted to our hospital from January 2017 to December 2017 were enrolled in this study.According to the random number table method,the patients were divided into an observation group and control group,62 cases each.The control group received routine health education before anesthesia on the day of surgery,and the observation group received pre-anesthesia centralized management.The psychological stress response,negative emotion,and anesthesia recovery time,and complications were compared between the two groups 1 day before the surgery,before the anesthesia,and 1 day after the surgery.Results:The Anxiety Score Scale(SAS)and The Depression Score Scale(SDS)scores of the observation group before the anesthesia and 1 day after the surgery were lower than those of the control group(P<0.05).The heart rate,systolic blood pressure,diastolic blood pressure,and prostacyclin levels of the observation group before the anesthesia and 1 day after the surgery were lower than those of the control group(P<0.05).The anesthesia recovery time and extubation time of the observation group were shorter than those of the control group(P<0.05),and the incidence of agitation and cognitive dysfunction were lower than those of the control group(P<0.05).Conclusions:The implementation of preoperative centralized nursing management could effectively alleviate the perioperative stress response and negative emotion of patients with tonsillectomy and adenoidectomy,which was conducive to improving the quality and safety of anesthesia recovery.
