数字化直接和间接印模法在全瓷冠临床应用中的比较研究

摘要
目的评价并比较数字化口内印模技术与传统精细印模技术在全瓷单冠修复中的临床效果。方法本研究将2016-12~2017-12在西安交通大学口腔医院综合科行二氧化锆全瓷单冠修复的74例患者纳入研究,其中35例采用3shape口内扫描仪直接法采集数据(直接组);39例采用硅橡胶制取印模后间接采集数据(间接组),完成最终修复,并记录取模和戴牙时间。戴牙后2周复诊时拟对患者进行满意度调查;戴牙完成2月后复诊,运用美国公共卫生署(USPHS)标准对修复体进行评价。结果共制作了135颗全瓷单冠,其中68颗为直接法,67颗间接法,直接组患者整体满意度高于间接组,在使用舒适度评价中直接法显著高于间接法,其差异有统计学意义(P<0.05);冠修复体边缘适合性评价中直接法显著优于间接法,其差异具有统计意义(P<0.05)。直接组平均取模时间长于间接组,其差异具有统计学意义(P<0.05);直接组平均戴牙时间小于间接组,其差异也具有统计学意义(P<0.05)。结论与间接法相比,直接数字化口内印模技术在全瓷单冠临床应用中效果更佳。
Objective To evaluate and compare clinical efficacy of all-ceramic zirconia crowns manufactured by direct digital intraoral impression versus silicone impression.Methods Thirty-five patients received oral impression via 3shape intraoral scanner,while 39 patients underwent oral impression via silicone impression technique.The time of impression and time of set-up were recorded.The patient's satisfaction was compared 2 weeks after treatment.The ceramic crowns were evaluated by USPHS standard 3 months after treatment.Results Total 135 all-ceramic crowns were finished,of which 68 were made by direct intraoral scan,and 67 by indirect scanner.Compared with indirect technique,the ‘comfort'in satisfaction scale and ‘marginal fitness' in USPHS scale were significantly increased in those with direct impressive technique(P < 0.05).Compared with indirect group,the mean time of set-up was significantly shorten,while the mean time of impression was significantly increased in direct group.Conclusion In this study,direct digital impressive technique(3shape trios) can provide a better clinical outcome and quality of medical service.
