生物反馈联合电刺激早期干预治疗产后盆底肌力减退的临床效果

摘要
目的探讨生物反馈联合电刺激早期干预治疗产后盆底肌力减退的临床效果。方法选取本院2017年2月至2018年2月收治的84例产后盆底肌力减退患者为研究对象,按照随机分组法将其分为观察组(42例)与对照组(42例)。对照组接受常规康复治疗,观察组在此基础上辅助生物反馈联合电刺激早期干预治疗。测定对比两组的产后盆底肌肉张力,并对比两组盆底肌肉张力分级情况、盆腔器官脱垂情况、尿失禁发生情况及并发症发生率。结果观察组患者产后3、6个月的阴道静息压、阴道收缩压及阴道收缩持续时间均明显优于对照组,差异具有统计学意义(P<0.05)。观察组治疗后6个月的盆底肌力Ⅲ~Ⅴ级患者占比明显高于对照组,差异具有统计学意义(P<0.05)。观察组产后3个月的盆腔器官脱垂0度患者占比为76.19%,明显高于对照组的47.62%(P<0.05)。观察组患者产后2、3、6个月尿失禁发生率均低于对照组,差异具有统计学意义(P<0.05)。观察组患者治疗后并发症的总发生率为4.76%,低于对照组的40.48%(P<0.05)。结论针对产后盆底肌力减退患者,可在常规治疗的基础上采用生物反馈联合电刺激进行早期干预治疗,其能够有效促进产妇盆底张力的恢复,降低盆腔器官脱垂等不良事件发生率,安全性更高。
        Objective To explore the clinical effect of early intervention of biofeedback combined with electrical stimulation in the treatment of postpartum pelvic floor muscle weakness. Methods Eighty-four patients with postpartum pelvic floor muscle weakness admitted in our hospital from February 2017 to February 2018 were selected as the research objects and divided into observation group(42 cases) and control group(42 cases) according to the random grouping method.The control group was given routine rehabilitation treatment, while the observation group received early intervention of biofeedback combined with electrical stimulation on this basis. The postpartum pelvic floor muscle tension were measured and compared in the two groups; the grade of pelvic floor muscle tension, pelvic organ prolapse, urinary incontinence and the incidences of complications of both groups were compared. Results The resting vaginal pressure, vaginal systolic pressure and duration of vaginal contraction in the observation group were significantly better than those in the control group at postpartum3 and 6 months(P<0.05). The proportion of patients with pelvic floor muscle strength of grade Ⅲ-Ⅴ in the observation group was significantly higher than that in the control group at 6 months after treatment(P<0.05). The proportion of patients with0 degree pelvic organ prolapse in the observation group at postpartum 3 months was 76.19%, which was significantly higher than 47.62% in the control group(P<0.05). The incidences of urinary incontinence in the observation group were lower than those in the control group at postpartum 2, 3 and 6 months(P<0.05). The total incidence of complications after treatment in the observation group was 4.76%, which was lower than 40.48% in the control group(P<0.05). Conclusion For patients with postpartum pelvic floor muscle weakness, early intervention of biofeedback combined with electrical stimulation on the basis of routine treatment can be used, which can effectively promote the recovery of pelvic floor tension and reduce the incidence of adverse events such as pelvic organ prolapse, with higher safety.
