双相躁狂发作期患者与健康人的尿酸、血脂对比分析

摘要
探讨双相情感障碍躁狂发作患者血清尿酸、血脂改变及临床意义,探讨双相躁狂发作患者血尿酸改变是否受季节、肾功、血脂的因素影响,为双相障碍的诊断治疗提供新的思路。通过回顾既往住院病例,纳入冬季(12月、01月、02月)双相情感障碍躁狂发作者47例,夏季(06月,07月,08月)双相情感障碍躁狂发作者47例,招募志愿者纳入冬季健康正常对照组56例,夏季健康对照组38例,应用微粒子发光技术测定次日空腹血清尿酸(UA)、肌酐(CRE)、甘油三酯(TG)及总胆固醇(CHO)水平。结果:冬季双相情感障碍躁狂发作组UA水平高于冬季健康对照组,差异有统计学意义(P<0.05);夏季双相情感障碍躁狂发作组UA水平高于夏季健康对照组,差异有统计学意义(P<0.01);冬季双相情感障碍躁狂发作组CHO水平高于夏季双相情感障碍躁狂发作组,冬季健康对照组CHO水平高于夏季健康对照组,差异均有统计学意义(P<0.05)。所有被试组尿酸水平与性别相关,男性尿酸水平高。整体水平中尿酸水平与血清CRE、TG、CHO水平呈正相关。结论:不同季节双相障碍躁狂发作患者血清尿酸水平均有升高且高于健康对照组,血清尿酸水平升高可能是双相情感障碍有意义的生物学指标,人群总胆固醇水平冬季可能高于夏季。
To investigate the changes of serum uric acid( UA),serum lipid and its clinical significance in patients with bipolar mania. We explore whether the changes of serum uric acid are affected by renal function,blood lipid and the seasonal changes to provide new thinking for the diagnosis and treatment of bipolar disorder. By reviewing previous electronic case records. 47 patients with bipolar mania attacked in winter( Nov,Jan,Feb) were enrolled and 47 patients with bipolar mania attacked in summer( May,Jun,July) were enrolled. By recruiting volunteers 56 cases of healthy persons in summer and 38 cases of healthy in winter were enrolled as control group. The fasting serum uric acid( UA),creatinine( CRE),triglyceride( TG) and total cholesterol( CHO) were measured in all participants by using Microparticle luminescence technique. UA levels in winter bipolar mania group were significantly higher than that in winter healthy control group( P < 0. 05). UA levels in summer bipolar mania group were significantly higher than that in summer healthy control group( P < 0. 01). CHO levels in winter bipolar mania group were significantly higher than that in summer bipolar mania group and CHO levels in winter healthy control group were significantly higher than that in summer bipolar mania group.( P < 0. 05). Serum uric acid levels of all the participants were correlated with sex. There was a positive correlation between uric acid levels and serum levels of CRE,TG and CHO. The levels of serum uric acid in patients with bipolar mania in different seasons are higher than those in healthy controls. The increase of serum uric acid level may be a significant biological index of bipolar mania. People's total cholesterol levels May be are higher than that in summer.
