二维和三维超声测量胎儿胸腺在胎儿宫内生长迟缓时的应用效果分析

摘要
目的通过二维超声、三维超声测量胎儿的胸腺体积、横径和前后径,以评估超声测量胸腺在胎儿宫内生长迟缓(IUGR)时的应用价值。方法选取2015年7月—2018年5月在该院妇产科接受超声检查,且疑诊为宫内生长迟缓的86例孕妇作为实验组,同时抽取86例相应孕周的胎儿生长正常的孕妇作为对照组,运用二维超声于三血管切面法测量胎儿的胸腺前后径、横径,采用三维超声多切面测量胎儿的胸腺体积,比较两组胎儿的前后径、横径和体积。结果两组胎儿的胸腺前后径、横径和体积随着胎儿孕周的增大而相应增大;而实验组IUGR胎儿的胸腺前后径、横径和体积则小于相应孕周的正常胎儿的相应超声测量值;针对两组胎儿的相应胸腺测量值进行t检验,实验组胎儿的前后径、横径、体积分别为(1.33±0.18)、(3.05±0.26)、(8.16±0.92),对照组的前后径、横径、体积分别为(1.41±0.21)、(3.29±0.32)、(8.60±0.98),实验组胎儿的胸腺前后径、横径、体积测量值明显低于对照组相应指标,且两两间比较差异有统计学意义(P<0.05)。结论胎儿的胸腺前后径、横径及体积会随着孕周的增大而增大,而当胎儿在母体子宫内生长迟缓时,其胸腺体积、前后径和横径均小于同龄胎儿的上述相应指标。
        Objective To evaluate the value of ultrasonography in measuring fetal growth retardation by measuring fetal thymus volume,transverse diameter and anterior and posterior diameter by two-dimensional and three-dimensional ultrasonography. Methods From July 2015 to May 2018, 86 pregnant women with intrauterine growth retardation were selected for ultrasound examination in the ob-stetrics and gynecology department of our hospital. And 86 pregnant women with normal fetal growth during the corresponding gesta-tion week were taken as control groups. The anteroposterior diameter and transverse diameter of the fetus were measured by two-di-mensional ultrasound in three-vessel section method. The thymus volume of the fetus was measured by three-dimensional ultrasound multi-section. The anteroposterior diameter, transverse diameter and volume of the two groups were compared. Results The diameter,transverse diameter and volume of fetal thymus increased with the increase of fetal gestation period. In the experimental group, the di-ameter, diameter and volume of the anterior and posterior thymus of the IUGR fetus were less than the corresponding ultrasound mea-surements of the normal fetus at the corresponding gestation period. The T test was performed on the corresponding thymic measure-ments of the two fetuses. The anterior and posterior diameter, transverse diameter and volume of the fetus in the experimental group were(1.33±0.18),(3.05±0.26) and(8.16±0.92), respectively. The anterior and posterior diameter, transverse diameter and volume of the fetus in the control group were(1.41±0.21),(3.29±0.32) and(8.60±0.98), respectively. The measurements of the anterior and posterior diameter, transverse diameter and volume of the thymus in the experimental group were significantly lower than those in the control group, and the difference between the two groups was statistically significant(P<0.05). Conclusion The anterior and posterior diameters, transverse diameters and volumes of the thymus of the fetus will increase with the increase of gestational weeks, and when the fetus grows slowly in the womb, its thymus volume, anterior and posterior diameters and transverse diameters are all smaller than those of the same age fetus.
