对子宫恶性肿瘤患者采取腹腔镜下广泛全子宫切除结合盆腔淋巴结清扫术治疗的疗效评价

摘要
目的研究对子宫恶性肿瘤患者采取腹腔镜下广泛全子宫切除结合盆腔淋巴结清扫术治疗的疗效。方法方便选择2016年1月—2018年1月该院收治的子宫恶性肿瘤患者82例,按照患者接受手术治疗的方式进行分组,41例患者接受腹腔镜下广泛全子宫切除结合盆腔淋巴结清扫术治疗,视作观察组,另外41例患者接受经腹广泛子宫切除术和淋巴结清扫术治疗,视作对照组,比较两组临床治疗效果。结果观察组术后手术各项情况均优于对照组;观察组术后尿潴留、输尿管狭窄、压力性尿失禁发生率分别为2.44%、2.44%、2.44%,对照组17.07%、14.63%、14.63%(χ2=4.9865、3.904 8、3.904 8,P=0.025 5、0.048 1、0.048 1);观察组术后性生活质量优良率为92.68%,对照组优良率75.31%(χ2=4.479 4,P=0.034 3)。结论腹腔镜下广泛全子宫切除结合盆腔淋巴结清扫术能够获得和经腹手术基本一致的临床效果,并且能够减轻手术创伤,加快手术进行,促进患者更迅速恢复,可在临床推广。
Objective To research the curative effect of laparoscopic Radical hysterectomy combined with pelvic lymphadenectomy in treatment of patients with malignant uterine tumors. Methods 82 cases of patients with malignant uterine tumors treated in our hospital from January 2016 to January 2018 were selected and divided into two groups with 41 cases in each, the observation group were treated with laparoscopic radical hysterectomy, while the control group were treated with transabdominal hysterectomy and lymphadenectomy, and the clinical treatment effect was compared between the two groups.Results After surgery, various indexes in the observation group were better than those in the control group, and the incidence rates of postoperative urinary retention, ureteral stricture and stress urinary incontinence in the observation group and in the control group were respectively 2.44%, 2.44%, 2.44% and 17.07%, 14.63%, 14.63%(χ2=4.986 5, 3.904 8, 3.904 8,P=0.025 5, 0.048 1, 0.048 1), and the excellent and good rate of sex quality of life after surgery in the observation group and in the control group was respectively 92.68% and 75.31%(χ2=4.479 4, P=0.034 3). Conclusion The clinical effect of laparoscopic Radical hysterectomy combined with pelvic lymphadenectomy in treatment of patients with malignant uterine tumors and transventral operation is basically consistent, which can relieve the operation wound, accelerate the development of operation, and promote the rapid recovery of patients, and it is worth clinical promotion.
