建立适合麻醉科的支气管镜消毒制度与质控标准

摘要
目的介绍适合麻醉科的支气管镜消毒制度和质控标准。方法 (1)胸外科手术间按照每个房间1台、10个手术间备用1台配置,推选便携式电子镜,价格便宜、耐用;(2)以国家卫生部门系列文件为依据制定支气管洗消流程;(3)专人负责洗消和质控,注意操作人员的职业防护;(4)新建与医院内窥镜室相同标准的麻醉科支气管镜洗消室,纳入医院感染管理系统内;(5)采用自制支气管悬挂装置保护支气管镜移动及使用中的安全。结果经该洗消方法后已有45 549例患者使用了支气管镜,无1例因消毒不当产生问题,洗消制度执行符合医院感染管理质控要求。结论麻醉科支气管镜的建设纳入医院内窥镜室同质化管理有一定的优势,自行设计一些辅助装置更适合麻醉科内镜的规范化应用。
Objective To introduce bronchoscopy disinfection system and quality control criterion suitable for department of anesthesiology. Methods( 1) The thoracic operating rooms are advised to equip with one set of flexible bronchoscopy in each room and one backup set in every 10 operating rooms. Moreover,the portable electronic bronchoscope is recommended for cheaper price and longer durability;( 2) Bronchoscopy disinfection process is based on a series of national health department regulations;( 3) Assign specific personnel responsibility for decontamination and quality control,and pay attention to the occupational protection for operators;( 4) New bronchoscope disinfection room inside department of anesthesiology is established to comply with the same disinfection management standard as rest of the endoscope rooms in the hospital;( 5) The self-made hanging device for bronchoscopes is used to protect bronchoscope from movement and to ensure the security during bronchoscopy. Results After adopting the fresh disinfection process,total of 45 549 patients were examined by bronchoscopy in our department,and none of them resulted in complications due to improper disinfection. The implementation of the disinfection system meets the requirement of quality control of hospital infection management. Conclusion There are certain advantages to incorporate bronchoscopes of anesthesiology department into equal endoscope management in the hospital. Self-designed auxiliary equipment such as hanging shelf facilitates the standardized application of anesthesia bronchoscopy.
