摘要
目的对罕见的抗-Jk~3合并抗-c E的标本进行鉴定。方法对被检标本分别做ABO血型、Rh血型和Kidd血型鉴定,并作Kidd血型的基因分型;尿素溶血实验以辅助鉴定Jk(a-b-)表型;血清学实验包括自身对照实验、直接抗人球蛋白实验和不规则抗体筛查;因实验室抗体鉴定用Jk(a-b-)表型红细胞为A型,故采用吸收放散实验鉴定标本是否有抗-Jk~。结果被检患者血型为B型,DCCee,自身对照阴性,抗体筛查阳性,直接抗人球蛋白实验阴性;血清学和基因分型鉴定标本kidd血型为Jk(a-b-);尿素溶血实验结果为尿素抵抗。标本检出有抗-Jk~3合并抗-cE抗体。结论对罕见的抗高频抗原抗体,应采用灵活的血清学技术手段并细致分析,以达到鉴定的目的。
        Objective To identify a rare sample which has anti-JK~3 combined with anti-cE. Methods The ABO blood group,Rh blood group and Kidd blood group were identified for the tested samples,and the Kidd blood group was genotyped.We performed a urea hemolysis test to assist in identifying the Jk( a-b-) phenotype. Serological experiments including selfcontrol experiments,direct anti-human globulin tests,and irregular antibody screening were conducted. Since the Jk( a-b-)phenotype erythrocytes were type A for laboratory antibody identification,we used the absorption and dispersion test to identify whether the sample had anti-Jk~3. Results The patient' s blood type was type B,DCCee,and self-control was positive.Tthe results of antibody screening were positive and DAT was negative. The kidd blood type of sample was Jk( a-b-) by serological and genotyping tests and urea hemolysis test showed urea resistance. The sample was confirmed to be anti-Jk~3 with anti-cE antibody. Conclusion For rare anti-high-frequency-antigen antibodies,flexible serological techniques should be adopted and analyzed meticulously to achieve the purpose of identification.
