三痹汤联合针刀及内热针对肩周炎患者疗效及其作用机制研究

摘要
目的分析三痹汤联合针刀及内热针对肩周炎患者疗效及其作用机制。方法搜集2015年3月至2018年3月期间于本院中医特色专科接受诊治肩周炎患者80例。按照随机数字表法分为对照组、观察组,各为40例。对照组行三痹汤治疗,观察组在对照组基础上联合针刀及内热针治疗。观察比较2组临床疗效。结果观察组总有疗效为90.00%,明显高于对照组的70.00%,(P<0.05);治疗前2组患者Constant-Murley评分比较无显著差异(P>0.05)。经1个月治疗后观察组康复各维度评分及总评分上升比对照组明显(P<0.05);且观察组TNF-α、IL-6、IL-8炎症因子水平明显比对照组低(P<0.05)。结论与单用三痹汤疗效比较,三痹汤联合针刀及内热针治疗肩周炎疗效更为突出,减少炎症因子水平,有助于缓解患者肩关节疼痛程度,提升关节功能康复,确保临床疗效,值得临床开展应用。
Objective To analyze the efficacy and mechanism of Sancha Tang combined with acupuncture knife and internal heat in the treatment of patients with frozen shoulder. Methods We collected 80 patients with periarthritis of shoulder from our hospital during March 2015 to March 2018. According to the random number table method,they were divided into a control group and an observation group,each with 40 cases. The control group was treated with San Bi Tang,and the observation group was combined with a needle knife and internal heat needle on the basis of the control group. Observe and compare the clinical efficacy of two groups. Results There was a total effect of 90.00% in the observation group,which was significantly higher than 70.00% in the control group(P<0.05). There was no significant difference in the Constant-Murley score between the two groups before treatment(P>0.05). After one month of treatment,the scores of recovery in the observation group and the increase of the total score in the observation group were significantly higher than those in the control group(P<0.05). The level of inflammatory cytokines of TNF-α,IL-6,IL-8 in the observation group was significantly lower than that in the control group(P<0.05). Conclusion Compared with the effect of single-use SanBi Tang,SanBi decoction combined with acupuncture knife and internal thermal acupuncture has a more prominent effect on the treatment of periarthritis of shoulder,reduce the level of inflammatory factors,help relieve the degree of shoulder pain in patients,improve joint function recovery,and ensure clinical efficacy,It is worthy of clinical application.
