心脏疾病妇女围产期发生心脏不良事件的相关因素研究

摘要
目的:探讨心脏疾病妇女围产期发生心脏不良事件的相关因素。方法:收治心脏疾病产妇100例,围产期未发生心脏不良反应的患者为对照组,围产期发生心脏不良反应的产妇为观察组。分析相关因素。结果:观察组患者年龄高于对照组,左心室射血分数(LVEF)低于对照组(P<0.05),两组妊娠次数、流产史与自然受孕率差异无统计学意义(P>0.05),观察组患者糖尿病史、妊娠高血压、肺动脉高压、深静脉血栓、NYHA>Ⅰ级的比例均显著高于对照组(P<0.05),而两组患者妇科炎症史差异无统计学意义(P>0.05)。Logistic回归分析结果显示,LVEF、肺动脉高压、深静脉血栓、NYHA>Ⅰ级均是围产期心脏不良事件发生的危险因素。结论:LVEF、肺动脉高压、深静脉血栓、NYHA>Ⅰ级是围产期心脏不良事件发生的危险因素,临床上应该对这类产妇格外加强监护,并采取相应措施,保障母婴安全。
Objective:To explore the related factors of cardiac adverse events in women with heart disease during perinatal period.Methods:100 parturients with heart disease were selected,the perinatal patients who had no adverse cardiac reactions were treated as the control group,the perinatal women who had adverse cardiac reactions were treated as the observation group,and we analyzed the related factors.Results:In the observation group,the age of the patients was higher than that of the control group,and the LVEF was lower than that of the control group(P<0.05).There was no significant difference in the number of pregnancies,the history of abortion and the natural conception rate between the two groups(P>0.05).In the observation group,the proportion of diabetes mellitus history,pregnancy hypertension,pulmonary hypertension,deep venous thrombosis grade one NYHA>Ⅰ grade were significantly higher than the control group(P<0.05),there was no significant difference in the history of gynecological inflammation between the two groups(P>0.05).Logistic regression analysis showed that LVEF,pulmonary hypertension,deep venous thrombosis and NYHA>Ⅰ grade were risk factors of adverse cardiac events in perinatal period.Conclusion:The LVEF,pulmonary hypertension,deep venous thrombosis and NYHA>Ⅰ grade were risk factors of adverse cardiac events in perinatal period,clinically,we should strengthen guardianship for these women and take corresponding measures to ensure the safety of mother and infant.
