摘要
目的研究热性惊厥患儿核因子-κB(NF-κB)和神经元特异性烯醇化酶(NSE)的表达水平与脑损伤、心肌损伤程度的关系。方法选取2018年3月至2018年9月在我院诊断为单纯性热性惊厥的51例患儿纳入SFS组,38例复杂性热性惊厥纳入CFS组,同期诊断为上呼吸道感染的37例患儿纳入对照和NSE水平、脑脊液中神经因子BDNF和S100B以及外周血中心肌酶谱CK-MB和LDH水平,并分析相关性。结果 3组患儿的脑脊液和血液中NF-κB和NSE水平差异有统计学意义(P <0.05),CFS组的上述指标均为最高。3组患儿脑脊液的神经因子水平和血液心肌酶组最高,对照组最低。NSE和NF-κB在血液中和脑脊液中均呈正相关(R=0.532和R=0.693)。脑脊液NF-κB水平与BDNF和SI00B水平均呈相关(R=0.710和R=0.422)。患儿血液中NF-κB水平与CK-MB和LDH水平亦呈正相关(R=0.454和R=0.582)。结论热性惊厥患儿NF-κB和NSE的表达水平与患儿脑损伤和心肌损伤水平呈正相关,可以考虑作为热性惊厥患儿脑损伤和心肌损伤的评估标志物。
        Objective To study the relationship between the expression of NF- κB and NSE and the degree of brain or myocardial injury in children with febrile convulsion. Methods A total of 51 children with simple febrile convulsion(SFC)in our hospital from 2017 June to 2018 June were assigned into SFC group and another38 children with complex febrile convulsion(CFC)into CFC group. The control group included 37 children with upper respiratory infection. The expression level of NF-κB and NSE was measured in the blood and cerebrospinal fluid of the subjects. Neuron factors,BDNF and S100 B,were measured in cerebrospinal fluid and the myocardial enzymes,CK-MB and LDH,were assessed. These indexes were compared among three groups and their correlation was analyzed. Results The significantly different NF-κB and NSE expression levels were observed among three groups(P < 0.05)and NF-κB and NSE expression was the highest in CFC group. The significant difference of neuron factors and myocardial enzymes was observed among three groups(P < 0.05)and CFC group showed the highest but the control group the lowest. The positive correlation of NSE and NF-κB was found in either blood or cerebrospinal fluid(R = 0.532 and R = 0.693). In cerebrospinal fluid,NF-κB expression was positively correlated with BDNF and S100 B(R = 0.710 and R = 0.422)and so was NF-κB and CK-MB or LDH(R = 0.454 and R =0.582). Conclusion NF-κB and NSE expression are positively correlated with the degree of brain or myocardial injury. Thus,they could be potential biomarkers to assess brain and myocardial injury in children with febrile convulsion.
