妊高症产妇产后并发下肢深静脉血栓的防治

摘要
目的探讨妊高症产妇产后并发下肢深静脉血栓(DVT)的防治意义。方法选择石家庄市妇幼保健院2016年1月至2019年1月收治的妊高症产妇300例,按照护理方法不同将产妇分为对照组施行常规护理与研究组施行综合护理,各150例。比较两组的产后DVT发生率,护理后患侧—健侧小腿周径差值、不良情绪评分、生活质量评分和护理满意度。结果研究组产后下肢DVT发生率1.33%(2/150)低于对照组7.33%(11/150);研究组护理总满意率98.00%(147/150)高于对照组90.00%(135/150),差异有统计学意义(P<0.05)。护理后第3天和第5天,研究组的患侧—健侧小腿周径差值均小于对照组;护理后第五天,两组SAS和SDS评分均低于护理前;研究组SAS和SDS评分均低于对照组;两组生活质量各维度评分均高于护理前;研究组生活质量各维度评分评分均高于对照组,差异有统计学意义(P<0.05)。结论综合护理可有效减少妊高症产妇产后DVT的发生,减轻小腿肿胀程度,有利于改善产妇心理状况和生活质量。
Objective To investigate the prevention and treatment of postpartum deep venous thrombosis(DVT) of lower extremities in pregnant women with pregnancy induced hypertension. Method 300 puerperants with pregnancy induced hypertension admitted to Hebei Maternity & Child Healthcare Hospital from January 2016 to January 2019 were selected. According to different nursing methods, the puerperants were divided into control group received routine nursing care and study group received comprehensive nursing care, 150 cases in each group. The incidence of DVT of lower extremity, the difference of circumference between affected side and healthy side, the bad mood score,the score of life quality and nursing satisfaction were compared between the two groups. Result The incidence of DVT of lower extremity in the study group was 1.33%(2/150) lower than that in the control group 7.33%(11/150);the total satisfaction rate of nursing in the study group was 98.00%(147/150) higher than that in the control group90.00%(135/150), with significant difference(P<0.05). On the 3 rd and 5 th day after nursing, the difference of calf circumference between the affected side and the healthy side in the study group was smaller than that in the control group; on the 5 th day after nursing, the scores of SAS and SDS in both groups were lower than those before nursing;the scores of SAS and SDS in the study group were lower than those in the control group; the scores of each dimension of quality of life in both groups were higher than those before nursing; the scores of each dimension of quality of life in the study group were higher than the control group, the difference was statistically significant(P<0.05). Conclusion Comprehensive nursing care can effectively reduce the occurrence of postpartum DVT, reduce the degree of leg swelling,and improve the psychological status and quality of life of pregnant women with pregnancy induced hypertension.
