Siebens领域管理模式在慢性病管理中的应用研究进展

摘要
对Siebens领域管理模式的目标、核心要素、发展,以及在慢性病管理中的应用领域进行系统综述,并提出在国内应用Siebens领域管理模式的展望,旨在为我国慢性病管理的研究及实践提供参考。
