单核细胞比率对前列腺肿瘤的诊断价值

摘要
目的探讨单核细胞比率(M%)对前列腺肿瘤的诊断价值。方法经直肠前列腺穿刺活检,794例前列腺肿瘤患者根据病理学诊断分为前列腺癌组和良性前列腺增生组。将年龄、M%、前列腺特异性抗原(PSA)、前列腺特异性抗原密度(PSAD)、WBC、前列腺总体积(TPV)纳入研究,进行单因素分析及Logistic多因素回归分析,确定最终的独立影响因素,并绘制ROC曲线探讨M%对前列腺癌的诊断价值。结果年龄、PSA、M%和TPV是引起前列腺癌的独立影响因素(P<0.05),M%诊断前列腺癌的最佳截断点是8.72%,其敏感度为70.7%,特异度为76.2%,阳性似然比2.95,阴性似然比0.38,阳性预测值72.8%,阴性预测值是78.9%,尤登指数0.47。结论 M%对诊断前列腺癌有提示意义,但存在一定的局限性,仍需结合其他指标。
