摘要
目的评估磷霉素氨丁三醇颗粒(美乐力)对老年尿路感染的治疗价值。方法选择2017年6月至2018年6月期间本院收治的106例老年尿路感染患者,随机分为对照组(53例)和观察组(53例)。对照组采用左氧氟沙星治疗,观察组采用磷霉素氨丁三醇颗粒治疗,观察两组临床疗效;同时,分析磷霉素氨丁三醇和左氧氟沙星对两组治疗前分离出致病菌的清除率,并评估致病菌对磷霉素氨丁三醇和左氧氟沙星的耐药性。结果观察组治疗总有效率为92.45%(49/53),显著高于对照组83.02%(44/53),组间差异具有统计学意义(P<0.05)。观察组和对照组治疗前尿液微生物培养共分离出病原菌119株,磷霉素氨丁三醇清除率为93.28%(111/119),左氧氟沙星清除率73.95%(88/119),差异具有统计学意义(P<0.05)。观察组和对照组治疗前分离出病原菌的药敏试验结果显示:磷霉素氨丁三醇耐药率为4.20%(5/119),左氧氟沙星耐药率为47.06%(56/119),差异具有统计学意义(P<0.05)。结论磷霉素氨丁三醇颗粒(美乐力)对老年尿路感染的治疗效果可靠,细菌清除率较高,且病原菌耐药性相对较低,临床应用价值较高。
Objective To evaluate the value of Fosfomycine Trometamol Granules in treating urinary tract infections in the elderly. Methods 106 elderly patients with urinary tract infection were randomly divided into control group(n=53) and observation group(n=53). The control group was treated with levofloxacin, while the observation group was treated with fosfomycin tromethamine granules, and the clinical efficacy of the two groups was observed. At the same time, the clearance rates of focimycin tromethamine and levofloxacin for the isolation of pathogenic bacteria before treatment were evaluated, and the resistance of pathogenic bacteria to fosfomycin tromethamine and levofloxacin was evaluated. Results The total effective rate of the observation group was 92.45%(49/53), which was significantly higher than that of the control group 83.02%(44/53). The difference between the two groups was statistically significant(P<0.05). 119 strains of pathogenic bacteria were isolated from urine of observation group and control group before treatment. The clearance rate of fosfomycin and ambroxol was 93.28%(111/119), and that of levofloxacin was73.95%(88/119). The difference was significant(P<0.05). The antibiotic susceptibility test of the pathogenic bacteria isolated from the observation group and the control group before treatment showed that the drug resistance rate of fosfomycin and levofloxacin was 4.20%(5/119) and 47.06%(56/119) respectively, and the difference was significant(P<0.05). Conclusion Fosfomycin tromethamine granules(melalex) has reliable therapeutic effect on urinary tract infections in the elderly, high bacterial clearance rate, and relatively low resistance of pathogenic bacteria, high clinical value.
