摘要
目的:明确川东北地区接受华法林抗凝治疗的患者群体中,华法林血药浓度与国际标准化比值(INR)的相关性,为临床合理用药提供参考依据。方法:2015~2017年川东北地区67例行心脏瓣膜置换术并服用华法林的患者,监测华法林血药浓度,测定INR值,分析两者相关性。结果:67例心脏瓣膜置换术后患者1个月内华法林平均血药浓度为(613. 7±149. 6)ng·ml~(-1)。华法林剂量与INR的相关性系数为0. 24(0. 1

        Objective: To investigate the correlation between warfarin plasma concentration and international normalized ratio (INR) in patients with heart valve replacement in the northeast of Sichuan to provide reference for clinical rational drug use. Methods: Totally 67 patients with heart valve replacement treated with warfarin were chosen for the study from 2015 to 2017 in the northeast of Sichuan. HPLC was employed to measure the plasma concentration of warfarin,and an ACL200 automated coagulometer was used to monitor INR. Results: The results showed that the mean plasma concentration was (613. 7 ± 149. 6) ng·ml~(-1) in the 67 patients.Meanwhile,the coefficient was 0. 24 (0. 1 < P < 0. 2) for warfarin dosage with INR; the coefficient was 0. 23 (0. 3 < P < 0. 5) for warfarin plasma concentration with INR. Conclusion: The correlation is poor between warfarin plasma concentration and INR in the northeast of Sichuan. Meanwhile,in order to adjust the dosage more objectively and accurately,warfarin plasma concentration monitoring is necessary and helpful for clinical rational drug use.
