胰岛细胞抗体和胰岛素自身抗体与急性病毒感染的相关性及其临床应用

摘要
目的探究免疫球蛋白G-胰岛细胞抗体(immunoglobulin G-islet cell antibodies,Ig G-ICA)和免疫球蛋白M-胰岛素自身抗体(immunoglobulin M-insulin autoantibody,Ig M-IAA)与急性病毒感染的相关性及其临床应用价值。方法回顾性分析2016年8月至2018年3月北京中医药大学东方医院收治的70例急性病毒感染患者的病例资料,并将其作为研究组,以同期无糖尿病且无感染性疾病的70例健康体检者作为对照组。两组均在空腹条件下取静脉血,检测两组受试者的糖脂代谢情况,补体结合胰岛细胞抗体(islet tibodies,CF-ICA)、Ig G-ICA阳性检出率,胃壁细胞抗体、甲状腺微粒抗体以及Ig M-IAA和Ig G-IAA的阳性检出率。结果两组受试者的血糖及血脂水平差异无显著性(P>0.05)。研究组Ig G-ICA、CF-ICA、Ig M-IAA、Ig G-IAA阳性检出率均显著高于对照组(P<0.05),检出胃壁细胞抗体及甲状腺微粒抗体的例数显著多于对照组(P<0.05)。结论急性病毒感染后患者血清中的IAA水平升高,以Ig M-IAA升高最为显著,推测Ig M-IAA的产生可能与病毒感染涉及免疫细胞激活的机制有关。
        Objective To explore the relationship between immunoglobulin G-islet cell antibodies(IgG-ICA), immunoglobulin M-insulin autoantibody(IgM-IAA) and acute viral infection, and explore their clinical application value. Method From August 2016 to March 2018, 70 samples of patients with acute viral infection in the Oriental Hospital of Beijing University of traditional Chinese Medicine were collected as the study group, and samples of 70 physical examiners without diabetic and infectious diseases at the same time were collected as the control group. Venous blood samples were taken from the two groups of subjects on an empty stomach. Blood glucose and lipid metabolism, complement fixing islet cell antibodies(CF-ICA), positive detection rate of IgG-ICA, gastric parietal cell antibodies, the positive detection rates of thyroid particle antibodies, IgM-IAA and IgG-IAA were detected in the two groups. Result The blood glucose and blood lipid levels were normal in both groups, and there was no significant difference between the two groups(P>0.05). The positive detection rates of IgG-ICA, CF-ICA, IgM-IAA, IgG-IAA in the study group were significantly higher than that in the control group(P< 0.05), and the cases of gastric parietal cell antibodies and thyroid particle antibodies were significantly higher than those in the control group(P<0.05). Conclusion The level of IAA in serum of patients was increased after acute viral infection, and the increase of IgM-IAA was the most significant. It is speculated that the production of IgM-IAA may be related to the mechanism of viral infection involves activation of immune cells.
