超声乳化摘除与囊外摘除结合人工晶体植入术治疗青光眼合并白内障的疗效分析

摘要
目的探讨超声乳化摘除与囊外摘除分别结合人工晶体植入术治疗青光眼合并白内障的疗效。方法回顾性的选取本院2016年1月-2017年1月笔者所在医院收治的青光眼合并白内障患者共110例(134眼),随机抽签方式分为实验组和对照组,实验组患者采用超声乳化摘除并人工晶体植入术进行治疗,对照组采用囊外摘除并人工晶体植入术进行治疗,对两组患者治疗前后的视力、眼压、术后并发症进行对比分析。结果治疗前两组患者视力对比无明显差异,P> 0.05,治疗后实验组视力(0.91±0.09)明显优于对照组(0.64±0.06),两组患者与治疗前对比有统计学差异(P <0.05);治疗前两组患者眼压对比无明显差异,P> 0.05,治疗后实验组眼压(14.39±2.21)明显低于对照组(17.39±2.05),两组患者与治疗前对比有统计学差异(P <0.05);本次研究手术并发症主要以角膜水肿、前房角积血、前房渗出为主,实验组总发生率5.45%较对照组12.73%低,但因样本数量较少,组间比较无统计学意义,P> 0.05。结论两种手术方式均可以有效的改善患者眼压及视力,但超声乳化摘除整体疗效更佳,建议临床参考使用。
Objective To evaluate the efficacy of phacoemulsification and extracapsular extraction combined with intraocular lens implantation in the treatment of glaucoma combined with cataract. Methods A total of 110 patients(134 eyes) with glaucoma and cataract admitted to our hospital from January 2016 to January 2017 were retrospectively selected. The random sampling method was divided into the experimental group and the control group. The patients in the experimental group were treated with phacoemulsification.The patients were removed and treated with intraocular lens implantation. The control group was treated with extracapsular extraction and intraocular lens implantation. Visual acuity, intraocular pressure, and postoperative complications were compared between the two groups before and after treatment. Results There was no significant difference in visual acuity between the two groups before treatment(P >0.05). Visual acuity(0.91±0.09)was significantly better in the experimental group than in the control group(0.64±0.06). There was a statistically significant difference between the two groups before treatment(P < 0.05). There was no significant difference in intraocular pressure between the two groups before treatment(P > 0.05). After treatment, the intraocular pressure(14.39±2.21)in the experimental group was significantly lower than that in the control group(17.39 ±2.05). There was a difference between the two groups before treatment. Statistical differences(P < 0.05); The surgical complications of this study were mainly corneal edema, anterior chamber hemorrhage, and anterior chamber exudate. The total incidence rate in the experimental group was 5.45% lower than the control group12.73%, but due to the sample numbers were few, and there was no statistical difference between the groups, P > 0.05. Conclusions Both surgical methods can effectively improve the intraocular pressure and visual acuity of patients, but the overall efficacy of phacoemulsification is better, which is recommended to use in clinical reference.
