摘要
目的探讨姜黄素(curcumin)对大鼠岛状皮瓣(skin flap)缺血再灌注损伤(lschemia-reperfusion injury)的保护作用。方法将105只SD大鼠随机平均分成3组(n=35),每组按术后6、10、20、30 h及7 d分为5个亚组(n=7)。对照组(Ⅰ组):不做缺血再灌注处理。缺血再灌注组(Ⅱ组):手术当天至术后7 d,每日腹腔内注射生理盐水0.4 mL/kg。姜黄素治疗组(Ⅲ组):将生理盐水替换为姜黄素注射液50 mg/kg。在以大鼠旋髂深血管皮支(deep circumflex iliac vessels)为蒂的背部岛状皮瓣缺血再灌注损伤模型上,分别测定皮瓣组织中超氧化物歧化酶(superoxide dismutase,SOD)、髓过氧化物酶(myeloperoxidase,MPO)活性,丙二醛(malondialdehyde,MDA)的含量。并对皮瓣进行图像分析,观察皮瓣存活率。结果与Ⅰ组进行比较,Ⅱ组MDA含量及MPO活性明显升高(P<0.01),SOD活性明显降低(P<0.01)。与Ⅱ组进行比较,Ⅲ组MDA含量及MPO活性明显降低(P<0.01),SOD活性表达明显升高(P<0.01)。术后7 d皮瓣存活率为Ⅰ组>Ⅲ组>Ⅱ组(P<0.01)。结论姜黄素具有抗炎、抗氧化、提高清除氧自由基能力的作用,减轻皮瓣缺血再灌注损伤,促进皮瓣存活。
        Objective To investigate the protective effect of curcumin against ischemia-reperfusion injury in rat island skin flaps. Methods A total of 105 Sprague-Dawley rats were randomly divided into three groups(n=35 for each group); each group was divided into five subgroups(n=7 for each group) according to the time after operation(6 h, 10 h, 20 h, 30 h, and 7 d). The control group(group Ⅰ) was not subjected to any ischemia-reperfusion treatment except an intraperitoneal injection of normal saline at a dose of 0.4 ml/kg daily from the day of operation to 7 days after operation. The ischemia-reperfusion group(group Ⅱ) was subjected to ischemia-reperfusion treatment in addition to the same postoperative treatment as group Ⅰ. The curcumin treatment group(group Ⅲ) was subjected to ischemia-reperfusion treatment in addition to the same postoperative treatment as group Ⅰ except that normal saline was replaced by curcumin injection(50 mg/kg). The activities of superoxide dismutase(SOD)and myeloperoxidase(MPO) and the content of malondialdehyde(MDA) were measured in a rat model of ischemia-reperfusion injury in the dorsal island flaps pedicled to the deep circumflex iliac vessels. The viability of the skin flaps was evaluated by analyzing the flaps. Results Compared with group Ⅰ, group Ⅱ had significant increases in both the content of MDA and the activity of MPO(P<0.01), but a significant decrease in SOD activity(P<0.01). Compared with group Ⅱ, group Ⅲ had significant decreases inboth the content of MDA and the activity of MPO(P<0.01), but a significant increase in SOD activity(P<0.01). The viability of the skin flaps 7 days after operation was highest in group Ⅰ, followed by group Ⅲ and then group Ⅱ, with significant differences observed between the groups(P<0.01). Conclusion Curcumin has anti-inflammatory and antioxidant effects, as well as the ability to enhance oxygen free radical scavenging, which together alleviate ischemia-reperfusion injury of skin flaps and improve their viability.
