白血病髓外浸润的临床特点及影像学表现

摘要
目的提高白血病髓外浸润的影像学诊断水平。方法回顾性分析已确诊的50例白血病患者髓外浸润的临床特点及影像学表现。结果 50例患者中,肺部浸润并感染20例,淋巴结浸润19例,胸腺浸润2例,颅内浸润2例,肝、脾浸润18例,肾脏浸润1例,直肠浸润1例。CT和MRI影像学表现:肺部支气管血管束增粗、小叶间隔增厚、肺内多发肺结节;淋巴结和胸腺增大;脑沟模糊、脑实质软组织肿块;肝、脾、肾脏增大,肝脏和直肠肿块等。结论结合临床特点,CT和MRI影像学检查能为白血病的诊断和预后评估提供重要依据。
Objective To improve the level of imaging diagnosis for leukemic extramedullary infiltration.Methods Fifty patients with leukemia extramedullary infiltration were retrospectively analyzed.Results Among 50 patients,there were 20 patients with pulmonary infiltration and infection,19 patients with lymph node infiltration,2 patients with thymus infiltration,2 patients with infiltrating brain,18 patients with liver and spleen infiltration,1 patient with kidney infiltration and 1 patient with rectum infiltration.The clinical features showed by CT and MRI included thickening of bronchovascular bundles and interlobular septum,nodules in lungs,lymph node enlargement,thymus enlargement,anfractuosity obscurity,brain parenchyma mass,liver and spleen enlargement,liver lump andrectum mass etc.Conclusion In combination with clinical features,CT and MRI was an important basis for diagnosis and prognosis of leukemia.
