利用互联网开展社区慢性病随访管理的效果评估

摘要
目的 :评估利用互联网开展社区慢性病随访管理的效果,为社区慢性病随访管理提供新思路。方法 :选取120名慢性病患者分为对照组和观察组,每组各60例。对照组中,男性31例,女性29例;平均年龄(60.25±10.79)岁;高血压患者23例,2型糖尿病患者19例,高血压合并2型糖尿病患者18例。观察组中,男性34例,女性26例;平均年龄(62.88±11.31)岁;高血压患者25例,2型糖尿病患者20例,高血压合并2型糖尿病患者15例。对照组患者接受家庭医生签约服务干预,观察组患者在此基础上接受互联网随访管理。干预6个月后比较干预前后两组患者血压、血糖、健康知识知晓水平、营养与健身知信行得分。结果 :干预后高血压患者的血压水平和糖尿病患者的血糖水平均低于对照组(P <0.05)。健康知识知晓率,营养与健身知信行得分高于对照组(P <0.05)。结论 :利用互联网开展社区慢性病随访管理有助于改善慢性病患者的血压和血糖控制水平,提高患者健康知识水平,提示互联网在慢性病随访管理中具有积极的作用。
Objective: To evaluate the effect of the follow-up management of chronic diseases in the community by Internet and to provide new ideas for the follow-up management of chronic diseases in the community. Methods: One hundred and twenty patients with chronic diseases were selected and divided into a control group and an observation group with 60 cases in each group. In the control group, there were 31 males and 29 females with an average age of(60.25±10.79) years old;23 patients with hypertension, 19 patients with type 2 diabetes mellitus and 18 patients with hypertension and type 2 diabetes mellitus were included. In the observation group, there were 34 males and 26 females with an average age of(62.88±11.31) years old, and there were 25 patients with hypertension, 20 patients with type 2 diabetes mellitus and 15 patients with hypertension and type 2 diabetes mellitus. The patients in the control group received the signed service intervention of family doctors, and the patients in the observation group received Internet follow-up management on this basis. After 6 months of intervention, the blood pressure, blood glucose, health knowledge awareness level, nutrition and fitness knowledge, attitude and behavior scores of the two groups were compared before and after the intervention. Results: After the intervention, the blood pressure level and blood sugar level of hypertensive and diabetic patients of the observation group were lower than those of the control group(P<0.05).The nutrition and fitness knowledge, attitude and behavior scores of the observation group were higher than those of the control group(P<0.05). Conclusion: Using the Internet to carry out community follow-up management of chronic diseases can help to improve the blood pressure and blood sugar control level of patients with chronic diseases and improve their health knowledge level, and it is suggested that the Internet plays an active role in the follow-up management of chronic diseases.
