改良根治术与保乳术在早期乳腺癌患者中的应用效果

摘要
目的对比改良根治术与保乳术在早期乳腺癌患者中的应用效果。方法方便选择2015年1月—2016年12月该院88例早期乳腺癌患者作为研究对象,以随机数表法将其分为对照组与研究组,每组44例。对照组采取改良乳腺癌根治术治疗,研究组采取保乳手术治疗。根据乳腺癌生命质量测定量表(FACT-B)评价两组术后3个月时的生存质量,对比两组生存率、局部复发率与远处转移率。结果研究组术后3个月时生理状态(18.8±1.2)分、情感状态(17.5±0.7)分、功能状态(18.3±1.2)分、附加关注条目(20.6±1.5)分与FACT-B量表总评分(93.7±5.4)分均高于对照组(14.2±2.0)分、(14.0±1.5)分、(12.6±1.5)分、(17.4±1.3)分、(76.8±7.8)分(t=4.526,3.985,5.658,4.656,5.265,P<0.05)。随访1年,研究组生存率100.00%、局部复发率4.55%、远处转移率0.00%与对照组100.00%、2.27%、0.00%对比差异无统计学意义(χ2=0.000、0.125、0.000,P>0.05)。结论改良根治术与保乳术治疗早期乳腺癌的近期疗效相当,但保乳术可以加快患者的康复速度,保证其生存质量。
Objective To compare the effects of modified radical mastectomy and breast conserving surgery in patients with early breast cancer. Methods 88 patients with early breast cancer were conveniently enrolled in our hospital from January2015 to December 2016. The patients were divided into control group and study group by random number table method,with 44 cases in each group. The control group was treated with modified radical mastectomy, and the study group was treated with breast-conserving surgery. According to the Breast Cancer Quality of Life Measurement Scale(FACT-B), the quality of life at 3 months after operation was evaluated. The survival rate, local recurrence rate and distant metastasis rate were compared between the two groups. Results The physiological state(18.8±1.2) points, emotional state(17.5±0.7) points,functional status(18.3±1.2) points, additional attention items(20.6±1.5) points and FACT-B at 3 months after operation of total score of the scale(93.7±5.4) was higher than that of the control group(14.2 ±2.0)points,(14.0±1.5)points,(12.6±1.5)points,(17.4±1.3)points, and(76.8±7.8)points.(t=4.526, 3.985, 5.658, 4.656, 5.265, P<0.05). After 1 year of follow-up, the study group had a survival rate of 100.00%, a local recurrence rate of 4.55%, a distant metastasis rate of 0.00%, and a control group of 100.00%, 2.27%, and 0.00%. There was no significant difference(χ2=0.000, 0.125, 0.000, P>0.05). Conclusion Modified radical mastectomy and breast-conserving surgery for the treatment of early breast cancer are equivalent, but breast-conserving surgery can speed up the recovery of patients and ensure their quality of life.
