苦柏颗粒冷敷对面部激素依赖性皮炎患者IL-4、IgE和皮肤屏障功能的影响

摘要
目的:分析苦柏颗粒冷敷对面部糖皮质激素依赖性皮炎(FCAD)患者皮肤屏障功能、免疫球蛋白E(IgE)及白介素-4(IL-4)的影响。方法:选取106例FCAD患者,随机分为对照组给予常规西医治疗,观察组给予苦柏颗粒冷敷治疗,两组各53例。治疗后1个月,评估两组患者临床疗效;比较两组患者治疗前后皮肤屏障功能指标、血清IgE和IL-4水平的差异。结果:两组患者治疗总有效率的比较,并无明显差异(P>0.05);但观察组治愈率较对照组明显升高(P<0.05)。相比治疗前,两组患者治疗后1个月面部皮肤油脂含量均明显升高,红斑总量均明显减少,经皮肤水分丢失(TEWL)值均明显下降(P<0.05);相比对照组,治疗后1个月观察组面部皮肤油脂含量明显升高,红斑总量明显减少,TEWL值明显下降(P<0.05)。相比治疗前,两组患者治疗后1个月血清IgE和IL-4的水平均明显下降,且观察组治疗后IgE和IL-4的水平较对照组明显下降(P<0.05)。结论:苦柏颗粒冷敷可有效调节FCAD患者皮肤屏障功能,改善体内IgE和IL-4的水平,临床治愈率较高。
Objective:To analyze the effect of Kubai granule cold compress on skin barrier function,immunoglobulin E(IgE) and interleukin-4(IL-4) in patients with facial corticosteroid-dependent dermatitis(FCAD).Methods:106 patients with FCAD.were randomly divided into control group treated with routine western medicine and observation group treated with Kubai granule cold compress,53 cases in each group.One month after treatment,the clinical efficacy of the two groups was evaluated,and the differences of skin barrier function,serum IgE and IL-4 levels between the two groups before and after treatment were compared.Results:There was no significant difference in the total effective rate between the two groups(P>0.05),but the cure rate of the observation group was significantly higher than that of the control group(P<0.05).Compared with before treatment,one month after treatment,the oil content of facial skin in both groups increased significantly,the area of erythema decreased significantly,and the trans-epidermal water loss(TEWL) value decreased significantly(P<0.05);compared with the control group,the oil content of facial skin in the observation group increased significantly,the area of erythema decreased significantly,and the TEWL value decreased significantly(P<0.05).Compared with before treatment,the levels of serum IgE and IL-4 in the two groups decreased significantly one month after treatment,and the levels of IgE and IL-4 in the observation group decreased significantly after treatment compared with the control group(P<0.05).Conclusion:Cool compress with Kubai granule can effectively regulate skin barrier function and improve IgE and IL-4 levels in patients with FCAD.The clinical cure rate is high.
