三角吻合术在腹腔镜远端胃癌根治术中的应用

摘要
目的探讨三角吻合术在腹腔镜远端胃癌根治术中的应用。方法随机选取我院在2015年1月-2017年1月收治的胃癌患者共60例作为研究对象进行分析研究,按照数字表法将其分为对照组30例及研究组30例,对照组给予常规腹腔镜辅助远端胃癌根治术,研究组给予全腹腔镜下远端胃癌根治术联合三角吻合技术,研究观察两组患者的临床疗效、手术指标和术后情况、并发症发生情况。结果研究组的临床疗效较对照组,有统计学意义(86.67%VS 30.00%,P<0.05);研究组的术中出血量、淋巴清扫数目少于对照组(P<0.05),研究组的手术时间、术后肛门排气时间短于对照组(P<0.05),研究组的术后疼痛(VAS)评分低于对照组(P<0.05);研究组的并发症总发生率较对照组,有统计学意义(20.00%VS56.67%,P<0.05)。结论三角吻合术联合腹腔镜远端胃癌根治术治疗胃癌的临床效果理想,安全性较高,可显著加快患者的康复进程,该种方案值得在临床上进一步推广。
Objective To explore the application of triangular anastomosis in radical laparoscopic radical gastrectomy.Methods A total of 60 patients with gastric cancer admitted from January 2015 to January 2017 were enrolled in the study.They were randomly divided into the control group and the study group,30 patients in each group.The control group received conventional laparoscopic assisted radical gastrectomy for radical gastrectomy.Laparoscopic radical gastrectomy combined with triangular anastomosis technique was used to study the clinical efficacy,surgical parameters,postoperative status and complications.Results The clinical efficacy of the study group was statistically significant compared with the control group(86.67% vs 30.00%,P<0.05).The intraoperative blood loss and lymphatic dissection in the study group were significantly lower than those in the control group(P<0.05).The operation time and postoperative anal exhaust time were significantly shorter than the control group(P<0.05).The postoperative pain(VAS) score of the study group was significantly lower than that of the control group(P<0.05).The total incidence of complications in the study group was higher.The control group was statistically significant(20.00% vs 56.67%,P<0.05).Conclusion Triangular anastomosis combined with laparoscopic radical gastrectomy for gastric cancer has a good clinical effect,high safety,and can accelerate recovery,which is worth further promotion.
