摘要
目的分析UF-1000i尿沉渣分析仪检测尿液管型存在的假阳性、假阴性因素,提出相应的解决方案。方法随机选取本院2018年1~4月门诊、住院尿液标本2130例分别用干化学分析法、UF-1000i分析仪法和人工显微镜法进行尿液有形成分检测,并收集有关管型的数据。以人工显微镜法判定管型检测真阳性、真阴性标准。结果 UF-1000i尿液分析仪法和显微镜镜检法的阳性率分别是11.60%、4.51%,两者比较差异有统计学意义(P <0.05)。UF-1000i分析仪法检测管型的假阳性率为7.09%,假阴性率为0.28%。引起UF-1000i假阳性的因素有黏液丝,各种结晶、细菌成团,白细胞、上皮细胞成团,分别占比例为60.93%、23.84%、15.23%。引起UF-1000i假阴性的因素为管型短小或溶解,或者标本的电导率低出现漏诊及管型内颗粒无法被荧光染料染色造成仪器漏检。结论UF-1000i尿沉渣分析仪检测尿液管型需结合显微镜镜检法,并进一步优化仪器性能以提高结果的准确性。
        Objective To analyze the false-positive and false-negative factors of urine castdetection by UF-1000 i urine sediment analyzer, and propose corresponding solutions. Methods A total of 2130 urine samples from outpatient and inpatient patients in our hospital from January to April 2018 were randomly selected. Dry chemical analysis, UF-1000 i analyzer and artificial microscopy were used to detect the urine formed elements, and the data of urine cast were collected. Artificial microscopy was used to determine the true-positive and truenegative criteria for urine cast test. Results The positive rates of UF-1000 i urine analyzer and microscopic examination were 11.60% and 4.51% respectively, the difference was statistically significant(P < 0.05). The false positive rate and false negative rate of UF-1000 i analyzer were 7.09% and 0.28% respectively. Mucous filaments, various crystals, bacterial clusters, white blood cells and epithelial cells were the main factors caused false positive of UF-1000 i, accounted for 60.93%, 23.84% and 15.23% respectively. The causes of UF-1000 i false negative were the cast was short or dissolved, or the conductivity of the specimen was low, and the missing specimens and the particles in the cast cannot be stained by fluorescent dyes. Conclusion The UF-1000 i urine sediment analyzer should be combined with microscopy to detect the urine cast, and further optimize the performance of the instrument to improve the accuracy of the results.
