水平肌加强减弱术联合下斜肌缩短术治疗斜视患儿的临床效果分析

摘要
目的研究小儿斜视采用水平肌加强减弱术联合下斜肌缩短术治疗的临床疗效。方法从郑州儿童医院2016年6月~2017年6月期间接诊的斜视患儿中,选取符合纳入标准的66例患儿到随机对照试验中,根据随机数字表分组法将患儿分为两组,每组33例。观察组行水平肌加强减弱术及下斜肌缩短术治疗,对照组行水平肌加强减弱术治疗,通过比较两组患儿的斜视症状改善情况和下斜肌功能恢复情况,评价水平肌加强减弱术联合下斜肌缩短术治疗小儿斜视的临床疗效。结果与对照组斜视症状改善总有效率84.85%相比,观察组的斜视症状改善总有效率96.97%更高,观察组患儿的下斜肌功能改善总有效率87.88%显著高于对照组患儿下斜肌功能改善总有效率75.76%,组间数据比较存在显著差异(P<0.05)。结论对于小儿斜视采用水平肌加强减弱术联合下斜肌缩短术治疗可取得理想的临床疗效,且有助于改善患儿下斜肌功能,其手术效果较单一使用水平肌加强减弱术更有效,可在临床应用。
Objective To study the clinical effects of strengthening and weakening of horizontal muscle combined with shortening of inferior oblique muscle in treatment of children with strabismus. Methods 60 cases of children with strabismus and who conformed to the inclusion criteria in Zhengzhou Children's Hospital from June,2016 to June,2017 were selected into the randomized controlled trial,and were divided into two groups according to the random number table group method, with 33 cases in each group.The observation group were treated with strengthening and weakening of horizontal muscle combined with shortening of inferior oblique muscle,while the control group were treated with horizontal muscle strengthening.Then,the improvement of the strabismus symptoms and the recovery of function of the inferior oblique muscle were compared between the two groups,and the clinical effect of strengthening and weakening of horizontal muscle combined with shortening of inferior oblique muscle in treatment of children with strabismus were evaluated. Results Compared with the control group of improvement of the total efficiency of strabismus symptoms was 84.85%,which the improvement of the total effective rate of the strabismus symptoms of 96.97% in the observation group was higher.The total effective efficiency of the lower oblique muscle of the children in the observation group was 87.88%,which was significantly higher than that in the control group of 75.76%, and there was significant difference in data between groups(P <0.05). Conclusion The strengthening and weakening of horizontal muscle combined with shortening of inferior oblique muscle in treatment of children with strabismus which can achieve an ideal clinical effect and it is help to improve the function of the inferior oblique muscle in the children,and the operation effect is more effective than the single use of horizontal muscle strengthening,thus it is worth promoting in clinical.
