经皮射频热凝消融术结合腰痹通胶囊治疗椎间盘源性腰痛的疗效观察

摘要
目的:探讨经皮射频热凝消融术结合腰痹通胶囊治疗椎间盘源性腰痛的临床疗效。方法:选取2013年8月—2018年2月该院收治的椎间盘源性腰痛患者82例为研究对象,随机分为研究组与对照组,对照组患者单纯给予经皮射频热凝消融术治疗,研究组患者给予经皮射频热凝消融术联合腰痹通胶囊治疗,治疗结束后评价两组患者的临床疗效,对比术前术后疼痛评分、Oswestry功能障碍指数(ODI)、改良JOA评分及炎症因子水平,并观察两组患者不良反应发生情况。结果:研究组患者的优良率显著高于对照组(P<0.05);治疗后,两组患者的VAS、ODI评分显著较治疗前降低,JOA评分显著较治疗前升高(P<0.05),且研究组患者的VAS、ODI评分低于对照组,JOA评分高于对照组(P<0.05);治疗后,两组患者的TNF-α、CRP、IL-6水平均较治疗前降低,且研究组患者的TNF-α、CRP、IL-6水平明显低于对照组(P<0.05)。结论:经皮射频热凝消融术结合腰痹通胶囊治疗椎间盘源性腰痛的疗效显著,能够有效改善患者功能障碍、缓解腰部疼痛,值得临床推广。
Objective:To investigate the clinical effect of percutaneous radiofrequency thermocoagulation combined with Yaobitong Capsule in the treatment of lumbar intervertebral disc pain.Methods:From August 2013 to February 2018,82 patients with discogenic low back pain were randomly divided into two groups:the study group and the control group.The patients in the control group were treated with percutaneous radiofrequency thermocoagulation only.The patients in the study group were treated with percutaneous radiofrequency thermocoagulation combined with Yubitong Capsule.After treatment,the clinical efficacy of the two groups was evaluated and the preoperative and postoperative pain scores were compared with the preoperative and postoperative pain score and ODI,modified JOA score and the level of inflammatory factors.Adverse reactions were observed in both groups.Results:The good rate of patients in the study group was significantly higher than that of the control group(P<0.05).After treatment,the VAS and ODI scores in the two groups were significantly lower than those before treatment and the JOA score was significantly higher than that before treatment(P<0.05) and the VAS and ODI scores in the study group were lower than those in the control group and the JOA score was higher than that of the control group(P<0.05).After treatment,the levels of TNF-α,CRP and IL-6 in the study group were significantly lower than those in the control group(P<0.05).Conclusion:Percutaneous radiofrequency thermocoagulation combined with Yaobitong Capsule is effective in the treatment of discogenic low back pain.It can effectively improve the patients′ dysfunction and relieve the pain in the waist.It is worthy of popularizing in clinic.
