摘要
目的固尔苏在早产儿生命支持中的临床应用价值。方法方便选取40例早产儿,时间为2015年8月—2018年12月期间,将入选者随机分为对照组(n=20,采取常规技术生命维持)与实验组(n=20,在常规技术生命维持基础上应用固尔苏),对治疗情况及相关指标进行观察。结果实验组患儿呼吸机辅助通气时间、用氧时间、住院时间分别为(17.02±2.55)h、(9.12±1.56)d、(25.31±5.35)d,均较对照组明显缩短(t=-4.33、-8.50、-11.39,P<0.05);实验组治愈率、转院率、死亡率分别为95.0%、5.0%、0.0%,治愈率较对照组明显要高,转院率、死亡率明显要低(P<0.05);实验组并发症发生率均较对照组明显要低(P<0.05);用药后6 h,实验组患儿血气指标,PaO_2、SaO_2及PaCO_2分别为(88.65±9.55)mmHg、(94.65±4.37)%、(45.25±8.56)mmHg,均较对照组明显要优(t=3.39、4.51、-2.72,P<0.05);实验组患儿治疗后肺氧合功能,OI、a/AP(O_2)分别为(12.25±4.56)、(0.26±0.07),均显著改善,且较对照组明显要优,差异有统计学意义(t=-3.63、4.21,P<0.05)。结论对早产儿预防性使用固尔苏,为早产儿的成长提供生命支持,提高存活率,减少并发症,改善预后,值得于临床广泛推广。
        Objective To study the application value of Curosurf in premature infant's life support. Methods A total of 40 premature infants treated from August 2015 to December 2018 in our hospital were convenient selected and randomly divided into control group(n=20) and observation group(n=20). The control group took the conventional life support, and the observation group took the conventional life support plus Curosurf. The therapeutic effect and index were observed. Results The assisted ventilation time, oxygen-uptaking time and hospitalization time in the observation group [(17.02±2.55)h,(9.12±1.56)d,(25.31±5.35)d] was significantly shorter than those in the control group(t=-4.33,-8.50,-11.39, P<0.05). The cure rate, transfer rate and death rate of observation group were 95.0%、5.0%、0.0%. Compared with control group, the cure rate of the observation group was high, transfer rate and death rate were low(P<0.05). The inciclence of complications in the experimental group was significantly lower than in the control group(P <0.05).The complication rate PaO_2,SaO_2,PaCO_2 were(88.65±9.55)mmHg、(94.65±4.37)%、(45.25±8.56)mmHg,in the observation group was significantly better than that in the control group(t=3.39,4.51,-2.72,P<0.05). The oxygenation function in the observation group OI、a/AP(O_2)were(12.25±4.56),(0.26±0.07),after treatment was significantly better than that in the control group.the difference was statistically significant(t=-3.63,4.21,P<0.05). Conclusion Curosurf can increase the survival rate, reduce the complication rate and improve the clinical prognosis of premature infants. It shall be widely applied in clinical practice.
