自由体位分娩对阴道分娩产程及母婴并发症的影响

摘要
目的研究自由体位分娩对阴道分娩产程及母婴并发症的影响。方法研究对象选取我院产科2017年1月至2017年12月收治的186例产妇,根据产妇分娩意愿将其分为试验组和对照组,各93例,分别采用自由体位分娩和传统卧位分娩,比较两组产妇分娩的各产程时间及母婴并发症情况。结果试验组的第一产程、第二产程和总产程均明显短于对照组(P<0.05);第三产程两组比较,无明显差异(P>0.05);试验组产妇的宫颈裂伤、会阴侧切发生率和并发症总发生率明显低于对照组(P<0.05),新生儿的并发症(新生儿窒息、吸入综合症)总发生率明显低于对照组(P<0.05)。结论自由体位分娩能有效缩短阴道分娩产妇产程时间,降低母婴并发症发生率,值得推广。
Objective To study the effect of free position delivery on vaginal delivery and maternal and infant complications. Methods A total of 186 puerperas from January 2017 to December 2017 in the obstetrics department of our hospital-were selected as the study objects. According to the wishes of puerpera, the puerperas were divided into experimental group and control group, with 93 cases in each group. The two groups were gvien the free position and the traditional position respectively. The duration of delivery and the complications of maternal and infant were compared between the two groups.Results The first stage of labor, the second stage of labor and total labor of the experimental group were significantly shorter than those of the control group(P<0.05); but the third stage of labor in the two groups had no significant difference(P>0.05).The incidence of cervical lacerations, perineal lateral incision and total complication rate in the experimental group were significantly lower than those in the control group(P<0.05). The total incidence of neonatal complications(neonatal asphyxia, inhalation syndrome) in the experimental group was significantly lower than that in the control group(P<0.05). Conclusion Free position delivery can effectively shorten the vaginal delivery maternal labor time, reduce the incidence of maternal and child complications, which is worth promoting.
