摘要
目的采用基于体素的节点中心度(DC)方法探讨弥漫性轴索损伤(DAI)患者脑功能网络连接拓扑属性的变化特点。方法搜集28例DAI患者及相匹配的28名健康志愿者(对照组)的脑部静息态功能磁共振成像(rs-fMRI)数据,使用SPM 8及DPARSFA在Matlab 2012a平台上经预处理后采用基于体素的DC(阈值为0.25)方法计算两组DC值,将DAI组差异脑区的DC值分别与临床参数进行相关性分析。结果与正常对照组相比,DAI组右侧额中回、右侧辅助运动区、双侧顶下小叶的DC值显著减低,而右侧海马、左侧枕下回的DC值显著增高(P<0.001,团块体积≥540 mm~3,Alpha Sim校正);DAI患者右侧海马、左侧顶下小叶的DC值与格拉斯哥昏迷量表(GCS)评分呈负相关(r=-0.381,r=-0.461),左侧枕下回与简易精神状态检查量表(MMSE)呈负相关(r=-0.389);右侧海马的DC值与残疾等级量表(DRS)评分呈正相关(r=0.470),右侧额中回的DC值与MMSE呈正相关(r=0.382)。结论 DAI患者脑功能网络中心度存在改变,有助于进一步地理解DAI患者伤后认知功能障碍的神经机制。
        Objective To investigate the changes of brain function network connectivity topological properties in patients with diffuse axonal injury(DAI) by using voxel-based node centrality(DC) method.Methods The f MRI data of brain in 28 patients with DAI and 28 healthy controls were collected.The pre-treatment of SPM8 and DPARSFA on Matlab2012 a platform was based on voxel-based DC(threshold value was 0.25).Methods The DC values of the two groups were calculated,and the DC values of the different brain regions of the DAI group were correlated with clinical parameters.Results Compared with the normal control group,the DC values of the right frontal gyrus,the right auxiliary motion zone,and the bilateral inferior parietal lobe in the DAI group were significantly decreased,while the DC values in the right hippocampus and the left suboccipital gyrus were significantly increased(P<0.001,mass volume ≥ 540 mm~3,Alpha Sim correction);DC values of the right hippocampus and left anterior lobule of DAI patients were negatively correlated with GCS score(r=-0.381,r=-0.461),left suboccipital The return was negatively correlated with MMSE(r=-0.389);the DC value of the right hippocampus was positively correlated with the DRS score(r=0.470),and the DC value of the right frontal gyrus was positively correlated with MMSE(r=0.382).Conclusion There is a change in the centrality of brain function network in patients with DAI,which is helpful to further understand the neural mechanism of cognitive dysfunction in patients with DAI.
