摘要
目的探讨扩散峰度成像应用于常规MRI阴性颞叶癫痫的诊断价值。资料与方法收集临床确诊而常规MRI阴性的17例颞叶癫痫患者,同时选择与癫痫患者相匹配的17例正常志愿者作为对照组,行扩散峰度成像序列扫描。测量并比较两组双侧海马的各向异性分数(FA)、平均扩散系数、平均峰度、平行峰度、径向峰度。结果与对照组相比,癫痫组患侧及健侧海马FA均显著下降,差异均有统计学意义(P<0.05);癫痫组患侧与健侧海马之间各指标差异均无统计学意义(P>0.05)。结论扩散峰度成像可对常规MRI阴性的颞叶癫痫患者脑结构改变提供定量信息,为颞叶癫痫的研究提供依据。
        Purpose To evaluate the diagnostic value of diffusion kurtosis imaging which is applied to evaluate conventional MRI negative temporal lobe epilepsy(TLE). Materials and Methods Seventeen patients who were clinically confirmed and negative through conventional MRI in TLE group and corresponding 17 volunteers in control group were collected and diffusion kurtosis imaging sequential scanning was performed. Fractional anisotropy, mean diffusivity, mean kurtosis, axial kurtosis and radial kurtosis values in bilateral hippocampi and temporal lobes were measured and compared between the two groups. Results Compared with hippocampi in control group, fractional anisotropy in diseased and normal temporal lobe in epilepsy group were all decreased significantly and the difference was of statistical significance(P<0.05). There was no statistical significance regarding difference in mean diffusivity, mean kurtosis, axial kurtosis and radial kurtosis of diseased and normal hippocampi in epilepsy group(P>0.05). Conclusion Diffusion kurtosis imaging could provide quantitative information of cerebral structure changes in patients with TLE with negative conventional MRI, offering basis for the study of TLE.
