基于“卫生费用核算体系2011”的四川省精神疾病治疗费用分析

摘要
目的:基于国际最新的卫生费用核算体系2011版(SHA 2011),对2016年四川省居民精神疾病的治疗费用进行测算与分析。方法:采用经合组织、欧盟统计局和世界卫生组织共同修订的卫生费用核算体系2011版。结果:四川省精神疾病的治疗费用总量为38.81亿元,治疗费用花费最高的是精神分裂症、分裂型障碍和妄想性障碍类精神疾病,治疗费用为18.45亿元,占47.55%;超过50%的治疗费用集中在45-70岁之间,60-65岁人群发生的治疗费用占比最高,占12.86%;发生在医院的治疗费用占比最高,高达98.73%;从服务功能上看,93.26%的治疗费用发生在住院服务上。个人医疗负担偏重,家庭卫生支出比重达到了53.15%。结论:四川省精神疾病患者的个人负担较重,建议重点关注老年人群精神疾病的防治、加强对严重精神疾病的预防和控制,同时增加基层的专科能力。
Objective: Based on the System of Health Accounting 2011(SHA 2011), it calculated and analyzed the medical expenses of mental disorders in Sichuan Province in 2016. Methods: SHA 2011 was applied. Results: The treatment expense for mental disease in Sichuan was 3.881 billion yuan. The treatment costs for schizophrenia, schizotypal disorder and delusional disorder mental illness were the top 3 treatment expenses for mental illness, accounting for 47.55%; 60-65 year olds of medical expenses accounted for the highest proportion of 12.44%; the treatment expenses in hospitals took the highest proportion, which was 98.73%; from the service function, 93.26% of the medical costs occurred in the hospital service. The burden of personal medical care was relatively heavy, and the proportion of the family was 53.15%. Conclusion: The individual burden of mental illness in Sichuan was heavy. It was suggested to focus on the prevention and control of mental illness in the elderly, enhance the prevention and control of schizophrenia and other serious mental diseases, and increase the specialized ability of the grass-roots level.
