数字化咬合分析系统对全口义齿修复后咬合情况的动态评估分析

摘要
目的:探讨数字化咬合分析系统在全口义齿修复患者咬合情况动态评估中的应用价值,为未来全口义齿修复治疗研究提供参考。方法:选取2016年2月-2018年2月就诊并接受全口义齿修复的60例患者,修复后即刻、修复后2周、6周、12周、24周,使用T-scan数字化咬合分析系统对其咬合情况进行动态评估,记录各时点患者的T-scan检测结果,检测指标包括咬合接触时间(Occlusion Time,OT)、累积最大牙合力(Maximum Cumulative,Max force)、力中心点(Center of Force,COF)义齿移位情况,同时采用问卷调查的方式调查修复后各时点患者对咀嚼功能的满意度。结果:随着义齿修复时间的延长,患者修复2周、6周、12周的OT、COF-X水平呈递减趋势,Maxforce、COF-Y水平均呈递增趋势,除COF-Y外,其他指标各时点比较,差异有统计学意义(P<0.05);而在修复24周后,患者各T-scan检测指标水平较12周均无明显变化,差异无统计学意义(P>0.05);有全口义齿佩戴经历患者的OT、COF水平低于无经历者,MAXforce与COF-Y水平高于无经历者,但组间各时点OT、COF-X、MAXforce比较,差异有统计学意义(P<0.05);COF-Y比较,差异无统计学意义(P>0.05);随着修复时间的延长,患者咀嚼功能整体满意度明显提高,差异有统计学意义(P<0.05)。结论:随着全口义齿佩戴时间的延长,患者OT缩短、Maxforce增加、COF向中心移动,咬合稳定性好且趋于平衡,咀嚼功能恢复理想,数字化咬合分析系统用于动态评估全口义齿修复患者咬合情况,可以量化患者咬合情况,为义齿修复的稳定性提供条件。
        Objective To investigate the application value of digital occlusion analysis system in the dynamic evaluation of occlusion situations after complete denture restoration, and to provide reference for future treatment of complete denture restoration. Methods 60 patients with complete denture restoration from February 2016 to February 2018 were selected. The occlusion situations were dynamically evaluated by T-scan digital occlusion analysis system after restoration for immediately,2, 6, 12, 24 weeks. The T-scan detection results [occlusion Time(OT),maximum cumulative force(Max force),center of force(COF),denture translocation] of patients at different time were recorded. The satisfaction to masticatory function of patients after restoration at different time was investigated by questionnaire. Results As the time of denture restoration prolonged,the OT and COF-X levels of patients showed decline trend after restoration for 2, 6, 12 weeks, the Max force and COF-Y levels showed increased trend, except for COF-Y, other indexes had statistical difference at different time(P<0.05); After restoration for 24 weeks, the levels of T-scan detection indexes in the patients had no signi? cant changes compared with those in the 12 weeks, and there was no statistical difference(P>0.05); The OT and COF levels of patients with wearing experience of complete denture were lower than those patients without experience, the Max force and COF-Y levels of patients with experience were higher than those patients without experience, and there was statistical difference in the OT, COF-X, Max force between the two groups at different time(P<0.05); There was no statistical difference in the COF-Y among groups(P>0.05); As the time of denture restoration prolonged, the satisfaction to masticatory function of patients showed increased trend(P<0.05).Conclusion As the denture wearing time prolongs, the patient's OT shortens, the Max force increases, the COF moves toward the center, the occlusion stability is good and tends to the balance, the masticatory function recovers ideally.The application of digital occlusion analysis system in the dynamic evaluation of occlusion situations after complete denture restoration is effective, the patients' occlusion situations can be quanti? ed, so as to provide conditions for the stability of the denture restoration.
