摘要
目的探讨不同程度罗哌卡因用于超声引导下肌间沟壁丛神经阻滞的应用效果。方法搜集于2017年8月—2018年8月期间在该接收的上肢手术患者128例作为重点对象,根据手术麻醉的不同浓度划分为参照组(0.3%浓度罗哌卡因)及治疗组(0.5%浓度罗哌卡因),且每组均分为64例,对所有上肢手术患者经不同浓度罗哌卡因麻醉后的应用效果给予观察与分析,并予以总结。结果治疗组上肢手术患者的尺神经、桡神经、正中神经、肌皮神经的麻醉效果均显著优于参照组,差异有统计学意义(P<0.05);治疗组手术患者的麻醉镇痛时间为(9.55±3.47)min、运动阻滞恢复时间为(9.01±3.47)min、麻醉起效时间为(12.57±1.14)min,差异有统计学意义(P<0.05);治疗组手术患者获得麻醉总有效率为63(98.44%),较参照组手术患者获得的麻醉总有效率55(85.94%)更胜一筹,差异有统计学意义(χ~2=9.641,P<0.05)。结论较0.3%浓度利多卡因而言,0.5%浓度罗哌卡因用于超声引导下肌间苟碧成神经阻滞的临床应用效果佳,值得推广。
        Objective To investigate the effect of different degrees of ropivacaine on ultrasound-guided intermuscular groove brachial plexus block. Methods A total of 128 patients received upper extremity surgery during the period from August 2017 to August 1818 were enrolled. The subjects were divided into the reference group(0.3% ropivacaine) and the treatment group(0.5% ropivacaine), according to the different concentrations of surgical anesthesia, 64 cases each group. The effects of different concentrations of ropivacaine anesthesia in patients with upper extremity surgery were observed, analyzed and summarized. Results The anesthetic effects of ulnar nerve, phrenic nerve, median nerve and musculocutaneous nerve in the upper limb surgery group of the treatment group were significantly better than those in the reference group. The difference was statistically significant(P<0.05). The anesthesia time of the patients in the operation group was(9.55±3.47) min, the recovery time of exercise block was(9.01±3.47) min, and the onset time of anesthesia was(12.57±1.14) min. The difference was statistically significant(P<0.05). The total effective rate of anesthesia in the treatment group was 63(98.44)%, which was better than that in the reference group 55(85.94)%. The difference was statistically significant(χ~2=9.641, P<0.05) and statistically significant. Conclusion Compared with 0.3% concentration of lidocaine, 0.5% concentration of ropivacaine is effective in the clinical application of ultrasound-guided intermuscular groove brachial plexus block, which is worthy of promotion.
