摘要
目的:探讨空气波压力治疗仪预防全麻术后下肢深静脉血栓(DVT)的效果。方法:择取在医院接受乳腺、甲状腺全麻手术治疗的94例患者,根据入院先后顺序将其分为对照组和观察组,每组47例,对照组患者术后接受常规护理干预,观察组在接受常规护理的基础上应用空气波压力治疗仪进行护理干预,观察对比两组患者术后下肢肿胀和疼痛的发生率、血流峰值速率最大值(V_(max))和血流峰值速率平均值(V_(mean))速率及住院天数。结果:术后观察组DVT、下肢肿胀和疼痛的发生率分别为2.13%、4.26%和2.13%,而对照组分别为14.89%、14.89%和17.02%,两组比较差异均有统计学意义(x~2=10.456,x~2=6.525,x~2=8.562;P<0.05);术后1 d观察组与对照组患者下肢V_(max)和V_(mean)速率比较无差异,经过7 d不同方式的干预,两组下肢V_(max)、V_(mean)数值较术后1 d均有明显降低,观察组下降幅度大,两组比较差异有统计学意义(t=6.574,t=6.718;P<0.05);且观察组术后住院天数较对照组明显缩短,其差异有统计学意义(t=10.158,P<0.05)。结论:全麻患者术后采用空气波压力治疗仪可有效预防DVT及相关并发症的发生率,对促进患者术后早日康复、提高其生活质量具有重要的临床意义。
        Objective: To explore the effect of air wave pressure therapeutic apparatus in preventing postoperative deep venous thrombosis(DVT) of lower limbs with general anesthesia. Methods: 94 patients who underwent mammary gland and thyroid gland surgery with general anesthesia were selected. And all of them were divided into control group(47 cases) and observation group(47 cases) according to the sequential registration number. The patients of control group received routine nursing intervention and ones of observation group received the nursing intervention of air wave pressure therapeutic apparatus on the basis of routine nursing. The incidence rates of swelling and pain of lower limbs, the maximum value of bleeding peak velocity(V_(max)), the mean value of bleeding peak velocity(V_(mean)) and length of stay of two groups were further observed and compared. Results: The incidence rates of DVT, swelling and pain of observation group(2.13%, 4.26%, 2.13%) were significantly lower than those of control group(14.89%, 14.89%, 17.02%)(x~2=10.456, x~2=6.525, x~2=8.562, P<0.05), respectively. The differences of Vmax and Vmean of lower limbs between observation group and control group postoperative 1 d were no significance. Through 7 d interventions with different methods, the V_(max) and V_(mean) of lower limbs of two groups postoperative 7 d were obviously lower than those postoperative 1d, respectively, and the descend range of observation was big, and the differences of descend range of V_(max) and V_(mean) between two groups were significantly(t=6.574, t=6.718, P<0.05). Besides, the length of stay of observation group was significantly shorter than that of control group(t=10.158, P<0.05). Conclusion: Patients with general anesthesia who receive air wave pressure therapeutic apparatus can prevent occurrence of DVT and incidence rates of relevant complications, and it has importantly clinical value in promoting operatively early rehabilitation of patients and improving their quality of life
