摘要
目的通过meta分析探讨体质指数(BMI)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)在糖尿病足(DF)发生中的作用。方法在PubMed、CNKI、VIP、CBM、万方等数据库中,检索关于糖尿病足发生的营养指标的文献,检索时限从2007年1月—2017年5月。由2名评价者根据纳入、排除标准独立筛选文献、评价质量、提取资料后采用RevMan 5.3软件进行meta分析,用固定效应模型或随机效应模型合并平均差值(WMD值)及其95%CI分析各营养指标,采用敏感性分析评估结果的稳定性,用漏斗图和失效安全数评估文献的发表偏倚。结果共纳入30篇文献,Meta分析结果显示,HDL-C(WMD=-0.18,95%CI:-0.27~-0.09,P<0.01)、LDL-C(WMD=0.37,95%CI:0.09~0.64,P<0.01),在DF组与非糖尿病足(NDF)组差异有统计学意义(P<0.05)。而BMI(WMD=-0.40,95%CI:-0.93~0.13,P>0.05)、TG(WMD=0.04,95%CI:-0.21~0.29,P>0.05)、TC(WMD=0.07,95%CI:-0.34~0.48,P>0.05)在两组间差异无统计学意义(P>0.05)。结论低HDL-C、高LDL-C在糖尿病足的发生中起到了促进作用。
        [Objective]To evaluate the effects of body mass index(BMI), triglyceride(TG), total cholesterol(TC), high density lipoprotein cholesterol(HDL-C) and low density lipoprotein cholesterol(LDL-C) on the occurrence of diabetic foot(DF) by meta analysis. [Methods] The following database such as PubMed, CNKI, VIP, CBM and WANFANG were searched to collect the literatures on effects of the nutritional indexes on the occurrence of DF, and the retrieval time was from January 2007 to May2017. According to the inclusion and exclusion criteria, two evaluators screened the literature independently, extracted the data and assessed the quality. The meta analysis was performed by Rev Man5.3 software. The nutritional indexes were analyzed by fi xed effect model or random effect model, pooled mean difference(WMD) and 95%CI, with stability evaluated by sensitivity analysis. Moreover, the funnel plot and fail-safe number were used to assess the published bias of articles. [Results] A total of30 articles were included. Meta analysis showed that there were statistically significant differences in HDL-C(WMD=-0.18, 95%CI:-0.27--0.09, P<0.01) and LDL-C(WMD=0.37, 95%CI:0.09-0.64, P<0.01) between DF group and non-diabetic foot(NDF) group,while there were no statistically significant differences i n BMI(WMD=-0.40, 95%CI:-0.93-0.13, P>0.05), TG(WMD=0.04,95%CI:-0.21-0.29, P>0.05) and TC(WMD=0.07, 95%CI:-0.34-0.48, P>0.05) between two groups.[Conclusion] Lower HDL-C and higher LDL-C have a positive impact on the occurrence of diabetic foot.
