摘要
目的:探讨正畸治疗中应用二膦酸盐对牙根吸收及牙移动的影响。方法:选取2014年8月—2016年8月我院收治的牙根发育不良的患者120例,根据治疗方法不同分成两组,各60例。对照组应用正畸治疗,研究组在对照组基础上联合二膦酸盐(戴上矫正器前在上颌中腭侧黏骨膜的地方注射二膦酸盐50μL,隔三天注射一次)治疗,比较两组牙根吸收值、治疗前后牙根吸收程度的分布以及治疗后不同时刻牙移动距离。结果:治疗后第4,8,12天,研究组牙根吸收值显著低于对照组,差异有统计学意义(P <0.05);治疗后研究组牙根吸收程度分布0级占比显著高于对照组,1级、2级、3级、4级占比低于对照组,差异有统计学意义(P <0.05);治疗后第4,8,12天研究组牙移动距离均显著小于对照组,差异有统计学意义(P <0.05)。结论:正畸治疗中应用二膦酸盐有助于降低牙根吸收的程度,减少牙移动,改善正畸治疗的效果,值得临床推广应用。
        Objective: To investigate the effect of bisphosphonate on root resorption and tooth movement in orthodontic treatment. Methods: 120 patients with root dysplasia admitted to our hospital from August 2014 to August 2016 were selected and divided into two groups according to different treatment methods, 60 cases each. The patients in the control group were given orthodontic treatment, and the patients in the study group were treated with bisphosphonate(injection of bisphosphonate 50 μL at the maxillary medial palate mucoperiosteum before putting on orthotic appliance, once every three days). The value of root resorption, the distribution of root resorption degree before and after treatment and the distance of tooth movement at different time points after treatment were compared between the two groups. Results: On the 4 th, 8 th and 12 th day after treatment, the value of root resorption in the study group was significantly lower than that in the control group, and the difference was statistically significant(P<0.05). The proportion of root resorption degree distribution grade 0 in the study group after treatment was significantly higher than that in the control group, and the proportions of grade 1, grade 2, grade 3 and grade 4 in the study group were lower than those in the control group, with statistically significant differences(P<0.05). The distance of tooth movement on the 4 th, 8 th and 12 th day after treatment in the study group was significantly smaller than that in the control group, and the differences were statistically significant(P<0.05). Conclusion: The application of bisphosphonate in orthodontic treatment can help reduce the degree of root resorption, reduce tooth movement and improve the effect of orthodontic treatment, which is worthy of clinical application.
