纤维桩和金属桩全冠修复后下颌第一磨牙残根的有限元应力分析

摘要
目的分析经金属桩和纤维桩全冠修复后下颌第一磨牙残根在静态及动态载荷下,剩余牙体组织应力分布形式和应力大小。方法利用有限元法分别建立下颌第一磨牙残根纤维桩核冠和金属桩核冠修复的模型,静态载荷为在牙冠表面施加与牙体长轴一致的225N的压力,动态载荷为225N的半正弦脉冲,历时1ms。记录2种载荷下各部位牙本质von Mises应力最大值以及最终时刻的牙本质von Mises应力。结果 2种载荷下,2种桩核系统修复后牙本质最大Von Mises应力都位于牙本质颈部近舌侧。动态载荷下,牙本质最大Von Mises应力超过100Mpa。2种载荷下,纤维桩修复后牙本质各部位的Von Mises应力基本都略小于金属桩修复。动态载荷下,根分歧区的应力明显增大,显著高于其他部位。结论动态载荷下颈部的应力值大于牙本质的抗折强度。金属桩修复的牙本质应力普遍大于纤维桩,纤维桩能较好地分散应力,可以考虑作为后牙残根的一种较好的修复方式。
        Objective To analyze the stress magnitude and distribution of residual dentin in maxillary first molar restored with fiber post and casting post under static and dynamic loads using three-dimension finite element methods. Methods A mandibular first molar model with residual root restored with fiber post and casting post was established by the three-dimensional finite element method. A static load force of 225 N and a half sine pulse force of 225 N lasting 1ms was respectively applied vertically to the occlusal surface of dental crown. The maximum von Mises stress and the last von Mises stress of various parts in tooth was respectively recorded under static and dynamic load status. Results Under static and dynamic conditions, the maximum Von Mises stresses of mandibular molarrepaired by two post systems were concentrated at the lingual cervical. Under dynamic loads, the maximum Von Mises stress exceeded 100 Mpa. Under both loading conditions, the Von Mises stress of various parts in mandibular molar restored with the fiber post was slightly less than that of tooth restored with the metal posts. Under dynamic loading, the stress significantly increased in furcation area, significantly higher than other parts. Conclusion Stress value under dynamic condition was greater than the bending strength of dentine. The stress of dentin after metal post restoration were generally greater than that of dentin after fiber post restoration, suggestingfiber posts could better distribute the stress,which can be considered as a good way to dental restoration.
