摘要
目的:将IQ-单光子发射计算机断层扫描(SPECT)静息门控心肌灌注显像在不同重建参数条件下所测的EF值,与超声心动和心脏磁共振显像所测值进行分析比较,以探讨重建条件对结果的影响,并筛选IQ-SPECT的最佳重建参数。方法:对在3个月内行心脏磁共振显像、超声心动检查和IQ-SPECT静息门控心肌灌注显像的25例患者进行回顾性分析。采用四种不同的重建参数,将所测EF值与心脏磁共振显像和超声心动法所测值分别进行分析比较检验。以心脏磁共振显像所得EF值为基准,采用Bland-Altman法比较不同方法和心脏磁共振显像所测EF值之间的偏倚程度。结果:心脏磁共振显像和心肌灌注显像所测的EF值要远小于超声心动所测值。随着迭代次数和子集的增加,IQ-SPECT所测得的ESV值逐渐增大,导致EF值逐渐变小。当重建参数为15子集、5次迭代时,所测的ESV、EDV和EF值与心脏磁共振显像所测值最为接近,其中EF值与心脏磁共振显像所测值的偏倚仅为2%。结论:不同方法所得的EF值存在差异,临床医师应根据经验进行修正。15子集、5次迭代为本研究所得的IQ-SPECT最佳重建参数。
        Objective: This study compared the impact of different reconstruction parameters on measuring EF in IQ-SPECT gated rest myocardial perfusion imaging(MPI) study. Ejection fraction values measured by echocardiography and cardiac magnetic resonance imaging(CMR) were analyzed as well, to screen the best reconstruction parameters of IQ-SPECT. Methods: 25 patients who performed CMR, echocardiography and IQ-SPECT gated rest MPI in 3 months were retrospective analyzed. EF values were measured by four reconstruction parameters and compared with that measured by CMR and echocardiography using paired t-tests. EF values measured by four reconstruction parameters of MPI were compared to that measured by CMR using Bland-Altman analysis. Results: EF values measured by MPI and CMR were much lower than that measured by echocardiography. EF values grew with the increase of iterations and subsets. In the condition of 15 subsets and 5 iterations, EDV, ESV and EF values were most close to that measured by CMR. The bias of EF was 2%. Conclusions: EF values measured by different reconstruction method show significant difference. Nuclear Medicine physicians should be aware of this phenomenon. In this study, the best reconstruction parameters were 15 subsets and 5 iterations.
