天津市新型结核病防治服务体系下结防机构人力资源现况调查及配置评价

摘要
目的了解新型结核病防治(结防)服务体系下天津市结核病防治机构人力资源现况及公平性配置情况。方法采用自制的人力资源调查表对2018年天津市结防机构防控人员人力资源一般情况进行调查,并对数据进行分析。结果天津市市辖16区结核病防治机构,2018年常住人口1 546.95万人。全市结防机构结防人员共111名,男女性别比1∶2.14,31~49岁占62.83%,本科和大专学历占79.65%,中级和初级职称占85.0%,工作年限≥11年的占64.60%。全市区级结防人员人均服务人口为14.87万人,市内六区、环城五区、郊五区服务人口分别为10.48、17.70、20.91万人;人均服务半径为0.59 km,市内六区、环城五区和郊五区服务半径分别为0.15、1.11和2.34 km;人均服务活动性肺结核患者数,市内六区、环城五区、郊五区服务患者数分别为27、55和52人。按照所辖人口、陆地面积、平均服务活动性肺结核患者数分布的基尼系数分别为0.23、0.50和0.17。结论天津市结防机构结防人力资源配置按服务人口和平均服务活动性肺结核患者数计算配置公平性较好,按服务面积略有不均衡的现象;在人员构成上存在年龄偏大、工作年限较长、高学历及高职称人员稀缺等问题。
        Objective To understand the current situation of human resource and its fairness allocation in tuberculosis control institutions under new tuberculosis control service system in Tianjin. Methods A self-made human resources questionnaire was used to investigate the general situation of human resources in tuberculosis control institutions of Tianjin in 2018, and the data were analyzed. Results There were 16 districts in Tianjin and permanent resident population was 15.469 5 million people in2018. There were 113 tuberculosis control personnel in Tianjin. The male-to-female ratio was 1:2.14, personnel aged 31-49 years old accounted for 62.83%, personnel with undergraduate and junior college education accounted for 79.65%, personnel with intermediate and junior titles accounted for 85.0%, and personnel with working life more than 11 years accounted for 64.60%.The per capita service population of district-level tuberculosis control personnel was 1 487 000 people, while the per capita service population of tuberculosis control personnel in six urban districts, five districts around the city, and five suburban districts was 1 048 000, 1 770 000 and 2 091 000 people, respectively. Per capita service radius was 0.59 km, while the service radius of six urban districts, five districts around the city, and five suburban districts was 0.15, 1.11 and 2.34 km, respectively.Per capita service number of patients with active pulmonary tuberculosis in six urban districts, five districts around the city, and five suburban districts was 27, 55 and 52, respectively. The Gini coefficient according to the population, land area and average service number of patients with active pulmonary tuberculosis under its jurisdiction was 0.23, 0.50 and 0.17 respectively.Conclusion According to the service population and average service number of active pulmonary tuberculosis patients, the human resources allocation in tuberculosis control institutions of Tianjin has good fairness. However, there is a slightly unreasonable allocation according to service area. In terms of personnel composition, there are some problems, such as older age, longer working life, and lack of personnel with higher education and higher professional titles.
