牵引过伸按压法配合椎弓根钉棒系统内固定治疗陈旧胸腰椎压缩性骨折临床研究

摘要
目的探讨牵引过伸按压法配合椎弓根钉棒系统内固定治疗陈旧胸腰椎压缩性骨折的临床疗效。方法选取自2015年1月至2018年1月沈阳市骨科医院收治的40例陈旧胸腰椎压缩性骨折患者为研究对象。采用随机数字表法将患者分为观察组与常规组,每组各20例。常规组采取椎弓根钉棒系统内固定术治疗,观察组配合实施牵引过伸按压法和椎弓根钉棒系统内固定进行治疗。比较两组患者治疗前后的疼痛情况、后凸Cobb’s角、Oswestry功能障碍指数(ODI)及椎体前缘高度百分比。结果治疗后,观察组患者疼痛评分、ODI、后凸Cobb’s角均明显低于常规组,椎体前缘高度百分比高于常规组,组间比较,差异均有统计学意义(P <0. 05)。结论牵引过伸按压法配合椎弓根钉棒系统内固定治疗陈旧胸腰椎压缩性骨折,可有效减轻患者疼痛,改善其骨折情况,便于患者尽快康复。
Objective To investigate the clinical effect of traction and hyperextension compression combined with pedicle screw rod system internal fixation in the treatment of old thoracolumbar compression fractures. Methods A retrospective study was performed on40 cases of patients with old thoracolumbar compression fractures who were admitted from January 2015 to January 2018. Patients were randomly divided into observation group and conventional group,with 20 cases in each group. The conventional group of patients was treated with pedicle screw system internal fixation,while the observation group of patients was treated with traction hyperextension compression combined with pedicle screw system internal fixation. Pain,Cobb' s Angle of kyphosis,Oswestry disability index( ODI) and vertebral anterior margin height were compared between the two groups before and after treatment. Results After treatment,the pain score,ODI and Cobb's Angle of kyphosis in the observation group were significantly lower than those in the conventional group,and the height of vertebral anterior margin was higher than that in the conventional group,with statistically significant differences( P < 0. 05).Conclusion Traction hyperextension compression combined with pedicle screw rod system internal fixation for the treatment of old thoracolumbar compression fractures can effectively reduce the pain of patients,improve the fracture situation,and facilitate the recovery of patients as soon as possible.
