导乐分娩配合自由体位助产对初产妇产程及自然分娩结局的影响

摘要
目的探讨导乐分娩配合自由体位助产对产妇产程及自然分娩结局的影响。方法收集本院2015年6月~2017年7月收治的待产产妇150例,按随机数字表法分为观察组与对照组,每组各75例。对照组给予常规助产,观察组则给予导乐分娩配合自由体位助产,观察并对比两组产程及自然分娩结局情况。结果观察组第一产程为(246.10±105.32)min、第二产程为(38.76±19.28)min、第三产程为(4.82±2.58)min、总产程为(301.47±105.20)min,均分别明显短于对照组的(382.20±197.20)min、(76.84±35.05)min、(8.65±5.84)min、(465.31±218.40)min(P<0.05);观察组各产程生理应激水平明显低于对照组(P<0.05);观察组自然分娩率为85.33%,明显高于对照组的61.33%(P<0.05)。结论导乐分娩配合自由体位助产能够有效缓解应激水平、缩短产妇产程、降低剖宫产率,促进自然分娩,具有较高的推广价值。
Objective To explore the effect of doula delivery with free position on labor process and natural childbirth outcomes. Methods A total of 150 primiparas hospitalized from June 2015 to July 2017 were divided into control group and observation group,with 75 cases in each group. The control group adopted conventional delivery,while the observation group received doula delivery with free position. Stage of labor and delivery outcomes were compared between two groups. Results The duration of first stage of labor( 246.10 ± 105.32) min,second stage of labor( 38.76 ± 19.28) min,third stage of labor( 4.82 ± 2.58) min and total stage of labor( 301.47 ± 105.20) min of observation group were significantly less than that of control group respectively [( 382.20 ± 197.20) min,( 76.84 ± 35.05) min,( 8.65 ± 5.84) min,( 465.31 ±218.40) min]( P < 0.05). Stress level in every stage of labor observation group was significantly less than that of control group( P < 0.05).85. 33% primiparas in observation group delivery naturally,significantly higher than 61.33% in control group. Conclusions Doula delivery with free position can effectively release stress level,shorten the stage of labor. reduce rate of cesarean section and promote natural childbirth.
