行动研究法在影像科护理人员对比剂过敏反应预防及处置能力培训中的应用

摘要
目的提高影像科护理人员正确预防并快速处置对比剂过敏反应的能力,以保障影像医疗安全。方法基于行动研究法的框架,按照明确问题、制订计划、展开行动、效果评估、反思改进等5个步骤,对护理人员进行对比剂预防及处置能力的培训,并采用柯式模型对培训效果进行评估。结果受训者对培训项目、方法、内容和讲义的满意度较高,达到95%以上;培训后,护理人员平均考核成绩由(62.25±7.67)分提高至(89.71±4.02)分,患者因严重过敏反应需转急诊的转诊率由66.5%降为26.0%,患者满意度由66.8%提升到90.2%,差异均有统计学意义(均P<0.01);结论基于行动研究法的培训能够提高受训护理人员满意度,有效提高培训成绩,促使受训者自觉运用所学知识,达到正确预防并快速处置对比剂过敏反应的目的,对影像医疗安全具有重要意义。
Objective To improve the ability of nursing staff for preventing and treating allergic reaction of contrast medium,so as to ensure the safety of medical treatment.Methods Based on the framework of action research methodology, the nurses were trained in the ability of prevention and treatment of contrast medium allergy according to the five steps of defining problems,formulating plans,launching actions,evaluating effects and reflecting on improvement; and the training effect was evaluated by using the Kirkpatrick model.Results Trainees had a high degree of satisfaction with the training project,method,contents and lecture notes,reaching more than 95%.After the training,the average assessment score of nursing staff increased from 62.25±7.67 to 89.71±4.02.The referral rate of patients with severe allergic reactions to the emergency department decreased from 66.5% to 26.0%,and the satisfaction rate of patients increased from 66.8% to 90.2%.The differences were statistically significant(all P<0.01).Conclusions The training based on the action research method can improve the satisfaction of trained nurses,effectively increase the training results,and it promotes trainees to consciously use the knowledge,prevent and treat the anaphylaxis of contrast medium,which is of great significance to the safety of medical treatment.
