改良PPH治疗重度痔脱垂临床研究

摘要
为探讨改良PPH治疗重度痔脱垂的疗效,将我院收治的120例重度痔脱垂患者随机分为观察组和对照组,各60例,观察组采用改良PPH治疗,对照组采用传统PPH治疗,对比2组临床疗效、手术时间、术中出血量、创面愈合时间、术后疼痛等并发症发生情况,以及随访情况。结果显示,观察组总有效率(100%)明显高于对照组(86.7%),手术时间、术中出血量、创面愈合时间、术后疼痛评分均明显少于对照组,术后肛缘水肿、尿潴留、复发率均低于对照组,满意度高于对照组,差异均有统计学意义,P <0.05。2组在术后大出血、吻合口狭窄、肛门坠胀及精细控便能力下降方面比较差异均无统计学意义,P>0.05。结果表明,与传统PPH相比,改良PPH治疗重度脱垂痔治愈率更好,术后并发症更轻,远期复发率更低,满意度更好,临床应用价值更高。
To investigate the effect of modified PPH for treating severe prolapsed hemorrhoids(SPH),120 SPH patients treated in author's hospital were randomly divided into observation group(60 cases,received modified PPH)and control group(60 cases,routine PPH),then both's clinical effect,operative time,intraoperative bleeding volume,wound-surface healing time,postoperative pain and complication occurence status,as well as follow-up finding were compared.As results,in total effective rate,in the score on operative time,intraoperative bleeding volume,wound-surface healing time,postoperative pain,in the incidence of ano-edge edema and uroschesis,as well recurrence rate,and in satisfactory degree observation group was respectively and significantly higher(100% vs 86.7%),less,lower and higher than control group,there were statistical difference between the two grops(all,P <0.05),only in postoperative massive bleeding,stoma stenosis,tenesmus of anus,and anal accurate controlled capacity dropped there were no statistical difference between the two groups(P >0.05).Resutls show that compared with routine PPH above modified one for SPH has more high curative rate,more slighter postoperative complication,more lower longterm recurrence rate,its satisfactory degree is more higher,thus its clinical application value is just more higher.
