重组人干扰素α2b不同剂量雾化吸入应用于儿童病毒性肺炎的效果对比分析

摘要
目的:对比分析病毒性肺炎患儿雾化吸入不同剂量重组人干扰素α2b的临床效果。方法:随机数字表法将2017年2月至2018年8月我科收治的93例病毒性肺炎患儿分为2组,20万U·kg~(-1)组47例予20万U·kg~(-1)/次重组人干扰组α2b雾化吸入,10万U·kg~(-1)组46例予重组人干扰素α2b每次10万U·kg~(-1)雾化吸入,治疗3 w,对比两组治疗总有效率及气促、啰音、咳嗽、发热等症状消失时间和住院时间,并使用酶联免疫吸附法检测两组患儿白细胞介素-6(Interleukin-6,IL-6)、C反应蛋白(C-reactive protein,CRP)及肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)等炎性因子治疗前后变化情况及安全性。结果:20万U·kg~(-1)组患儿住院、气促、啰音及咳嗽消失时间和退热时间均少于10万U·kg~(-1)组(P<0.05),3 w末20万U·kg~(-1)组患儿血清CRP、IL-6及TNF-α水平均低于10万U·kg~(-1)组(P<0.05),两组总有效率及不良反应情况无统计学意义(P>0.05)。结论:病毒性肺炎患儿雾化吸入重组人干扰素α2b可显著改善其炎症状态及咳嗽、气促等体征,其中每次20万U·kg~(-1)用量较10万U·kg~(-1)更有效且较安全。
        Objective: To compare the effects of aerosolized different doses of recombinant human interferon α2b in children with viral pneumonia. Methods: Randomized digital table method was used to divide 93 children with viral pneumonia admitted to our department from February 2017 to August 2018 into two groups. In the 200,000 U·kg~(-1) group(n=47) received inhalation of 200,000 U·kg~(-1) recombinant human interference group α2b. The 100,000 U·kg~(-1) group(n=46) received 100,000 U·kg~(-1) for 3 weeks. The time of disappearance of symptoms such as shortness of breath, voice, cough, fever, and hospitalization time was compared. The expression of interleukin-6(IL-6), C-reactive protein(CRP), tumor necrosis factor-α(Tumor necrosis factor-α, TNF-α) and other inflammatory factors before and after treatment was detected by enzyme-linked immunosorbent assay. Results: The disappearance time and fever time of hospitalization, shortness of breath, voice and cough were lower of the 200,000 U·kg~(-1) group than that of the 100,000 U·kg~(-1) group. The levels of CRP, IL-6 and TNF-α of the 200,000 U·kg~(-1) group was significantly lower than those of 100,000 U·kg~(-1 )group(P<0.05). The total effective rate and adverse reactions of the two groups were not statistically significant(P>0.05). Conclusion: Inhalation of recombinant human interferon α2b in children with viral pneumonia can significantly improve the inflammation and signs of cough and shortness of breath, while the amount of 200,000 U·kg~(-1) is more effective than 100,000 U·kg~(-1).
