2008-2017年贵州省学生肺结核流行特征分析

摘要
目的了解贵州省2008-2017年学生肺结核流行特征,为学校结核病防控工作提供参考依据。方法收集2008-2017年"结核病管理信息系统"登记的肺结核病例资料进行描述流行病学分析。结果 2008-2017年贵州省共登记治疗学生肺结核29 238例,年均登记学生肺结核2 924例,年均登记率为33.12/10万。全省学生肺结核登记治疗的高峰集中在3-5月,分别占11.76%、12.46%、9.00%,毕节市登记治疗的学生肺结核患者最多,占全省学生病例总数的31.16%;全省学生肺结核男女之比为1.28∶1,主要集中在15～19岁这个年龄段,占61.67%。发现方式主要是以因症就诊(43.35%)和转诊(29.88%)为主,新涂阳学生肺结核患者治疗成功率为95.65%、复治涂阳学生肺结核患者治疗成功率为92.31%。结论 2008-2017年贵州省登记治疗的学生肺结核总体趋势较为平稳,应结合辖区学生实际情况,有针对性、实效性地制定防控策略,完善主动筛查机制,及时发现传染源,避免发生学校结核病聚集性的隐患。
Objective To understand the characteristics of tuberculosis epidemic among students in Guizhou province from2008 to 2017, and thus to provide references for school tuberculosis prevention and control. Methods Data of tuberculosis cases registered in the "TB management information system" from 2008 to 2017 were collected for descriptive epidemiological analysis. Results From 2008 to 2017, a total of 29 238 cases of students with tuberculosis were registered in Guizhou Province, with an average annual registration rate of 33.12/100 000(2924 cases). The peak of tuberculosis registration and treatment for students in the province were from March to May, accounting for 11.76%,12.46% and 9.00%, respectively.Students registered for treatment in Bijie city had the most tuberculosis patients, accounting for 31.16% of the total number of students in the Province. The ratio of male to female students with tuberculosis was 1.28:1, mainly in the age group of 15 to19 years old, accounting for 61.67%. The discovery methods were mainly based on the treatment(43.35%) and referral of patients with the disease(29.88%), and the treatment success rate of the new smear students' pulmonary tuberculosis patients was 95.65%, and the re-treatment success rate of the smear students' pulmonary tuberculosis patients was 92.31%.Conclusion From 2008 to 2017, the overall trend of tuberculosis in the students enrolled for treatment in Guizhou Province was relatively stable. In combination with the actual situation of students under the jurisdiction, the prevention and control strategies are formulated with pertinence and effectiveness, the active screening mechanism is improved, the source of infection is found in time, and the hidden danger of school tuberculosis aggregation is avoided.
